摘　　要：目的 :探讨热休克蛋白 70 (HSP70 )在肝外胆管癌组织中的表达及其临床意义 .方法 :以兔抗人HSP70多克隆抗体为试剂 ,应用免疫组织化学方法 (SP法 ) ,检测 30例肝外胆管癌组织中HSP70的表达水平 ,并与癌旁非癌组织作对照 .结果 :在肝外胆管癌 30例中 ,HSP70表达阳性率为 77% ,而癌旁非癌组织阳性率则为 13% ,二者有显著性差异 (P <0 .0 5 ) .癌组织中HSP70阳性表达率与病理分级有关 ,组织学分级Ⅰ ,Ⅱ级与Ⅲ ,Ⅳ级者阳性率分别为 5 6 %和 10 0 % ,二者有显著性差异 (P <0 .0 5 ) .原发癌灶直径≥ 2cm及 <2cm者HSP70阳性表达率分别为 95 %及 5 0 % ,二者有显著性差异(P <0 .0 5 ) ;肿瘤伴转移者与无转移者HSP70阳性表达率分别为 10 0 %及 5 9% ,二者有显著性差异 (P <0 .0 5 ) .结论 :肝外胆管癌组织HSP70表达明显增高 ,且与组织学类型、癌灶大小及癌转移有关 .AIM: To study the expression of HSP70 in human extrahepatic bile duct carcinoma and its significance. METHODS: By using polyclone antibody, the cellular distribution of HSP70 was performed in surgically removed human extrahepatic bile duct carcinoma and adjacent noncancerous tissues by immunohistochemical method. RESULTS: The positive expression rate of HSP70 was 77% in extrahepatic bile duct carcinoma tissues but was only 13% in adjacent noncancerous tissues. There was a significant difference between them ( P <0.05). The positive expression rate of HSP70 was significantly different ( P <0.05) between histological gradesⅠ, Ⅱ (56%) and Ⅲ, Ⅳ (100%) of extrahepatic bile duct carcinoma. HSP70 expression rates of bile duct carcinoma with the diameter of primary lesion ≥2 cm or <2 cm were 95% or 50% respectively ( P <0.05). Compared with that in non metastatic group, the HSP70 positivity in metastatic group was significantly higher (100% vs 59%, P <0.05). CONCLUSION: HSP70 is overexpressed in human extrahepatic bile duct carcinoma and is positively correlated with tumor differentiation, tumor size and metastasis. HSP70 can be helpful to the diagnosis and discrimination of bile duct carcinoma.
