摘 要:目的 :探讨新生儿窒息后水电解质和酸碱平衡紊乱及其与肾功能损害的关系。方法 :应用全自动生化分析仪测定窒息组 2 5例和对照组 12例的血清电解质、阴离子隙 (AG)、标准碳酸氢盐 (HCO3 -)、血浆渗透压(OSM)、血尿素氮 (BUN)和血肌酐 (SCr)浓度。结果 :重度窒息组血钾高于对照组。窒息组血钠低于对照组 ,其中重度窒息组又低于轻度窒息组。重度窒息组血钙低于对照组和轻度窒息组 ,而血磷则高于对照组和轻度窒息组。重度窒息组血尿素氮、肌酐、阴离子隙均高于对照组和轻度窒息组。窒息组血HCO3 -和血浆渗透压均低于对照组 ,其中重度窒息组又低于轻度窒息组。相关分析表明 ,血K+ 与HCO3 -呈负相关 ,与BUN呈正相关。血Na+ 与AG和OSM呈正相关 ,与BUN ,SCr呈负相关。血Cl-与HCO3 -、AG呈负相关 ,与OSM呈正相关。血Ca2 + 与BUN ,SCr呈负相关 ,与OSM呈正相关。血P-与AG ,BUN ,SCr呈正相关 ,与HCO3 -,OSM呈负相关。结论 :新生儿窒息后出现高钾、低钠、低钙、高磷血症及高AG型代谢性酸中毒 ,血生化、代谢紊乱与肾功能受损密切相关。Objective:To evaluate relationship between electrolyte, amion gap(AG) and renal function injury post asphyctic newborn.Methods:Serum electrolyte, AG, standard carbonate(HCO 3 -), plasma osmotic pressure (OSM), blood urea nitrogen(BUN) and serum creatinine(SCr) concentration were measured in 12 control neonates and 25 asphyctic newborns by automatic biochemical assay. Results:Serum potassium levels in major asphyxia group were significantly higher than those of control group( P <0.01). Serum sodium levels in asphyxia groups which were lower in major subgroup than those of gentle subgroup were significantly lower compared with those of control group ( P <0.01). Serum calcium, HCO 3 - levels and OSM in major asphyxia subgroup were significantly lower than those of gentle asphyxia subgroup and control group, but serum phosphorus, BUN, SCr and AG levels in major asphyxia subgroup were significantly higher than those of gentle asphyxia group and control group. There was high positive linear correlation between serum potassium levels and BUN, serum sodium and AG or OSM, serum phosphorus levels and AG or BUN or SCr. But there was high negative linear correlation between serum potassium levels and HCO 3 -, serum sodium levels and AG or OSM,serum chloridion levels and HCO 3 - or AG, serum calcium levels and BUN or SCr, serum phosphorus levels and HCO 3 - or OSM. Conclusions:There were hyperkalemia, hyponatremia,hypocalcemia, hyperphosphoremia and metabolic acidosis with hyperamion gap post asphyctic newborn, and renal function injury post asphyctic newborn was closely related with serum electrolyte and metabolic disorder.
