摘　　要：目的:探讨应用改良加长胸三角皮瓣同期修复全鼻及部分面颊大面积软组织缺损。方法:I期手术胸三角皮瓣的设计与手术按常规胸三角皮瓣的设计与手术,Ⅱ期改良加长胸三角皮瓣手术,通过胸三角皮瓣胸内侧蒂部向对侧胸部加长,4周后延迟断蒂,可使胸三角皮瓣加长5cm或更长,扩大了颜面颈部软组织缺损修复的适应证,使面颈部大面积软组织缺损同期修复成为可能。结果:应用改良加长胸三角皮瓣,对面中部全鼻缺失及部分眶下、颊部软组织大面积缺损的患者进行了同期修复与重建,获得了良好的临床应用效果。结论:这一手术方法有重要的临床意义及应用价值,值得推广应用。Objective: To study reconstruction for big soft tissue defect of the total nose and partial facial cheek with a modified lengthened deltopectoral flap in same - stage. Methods: The design and operation methods of deltopectoral flap were acted on the rule methods of deltopectoral flap operation. The method of modified lengthened deltopectoral flap was first operated by lengthening pedicel of deltopectoral flap to opposite side of breast, and then delayed cutting of pedicel after four weeks. It was lengthened for five centimeter or more and might repair a big soft tissue defect of the face and cervix. Results: Using a modified lengthened deltopectoral flap, authors performed an operation for a patient with a big soft tissue defect of the total nose and partial facial cheek. A fine clinical result was achieved. Conclusion: The modified lengthened deltopectoral flap has an important clinical significance and application value. It might repair a big soft tissue defect of the face and cervix.
