摘 要:目的 探讨螺旋CT血管造影 (SCTA) 3D重建图像技术在鞍区肿瘤治疗中的应用价值。方法 本组 2 0例鞍区肿瘤经MRI明确诊断后 ,行SCTA螺旋CT重建 3D图像。螺旋扫描层厚 2 .0mm ,间距 1.0mm ,螺距 1.0~ 1.2 5 ,以 3.0ml·s-1速度注射对比剂 12 0ml;用同遮盖法 (SSD)重建三维图像 ,所建图像与MRI及手术对照。 9例行伪彩SSD重建用以模拟手术入路。结果 螺旋CT 3D重建图像能清楚显示鞍区肿瘤的形态及其与大血管、颅骨的三维关系并可显示邻近受压移位的血管 ;螺旋CT重建图像技术可为模拟手术入路提供重要信息。结论 作为常规二维影像的补充 ,螺旋CT重建图像技术可提供鞍区肿瘤与邻近血管及颅骨的三维空间图像 ,对临床治疗有重要指导价值。Objective To investigate the clinical value of spiral CT angiography (SCTA) and 3D surface reconstruction applied in patients of sellar area neoplasms. Methods Twenty patients with neoplasms near the sellar area were examined with SCTA and MRI. The findings of SCTA were compared with those of MRI and in operation. The thickness of spiral scanning was 2.0 mm, index 1.0mm, pitch 1.0~1.25. One hundred twenty millilitre contrast mediums were injected intravenously at the rate of 3.0 ml·s -1 . The surface shaded display (SSD) technique was used to reconstruct the 3D images in all cases and the color SSD images were used to imitate the approach of operation in 9 cases. Results The 3D images of SCTA clearly demonstrated the 3D shapes of the neoplasms and their relationship with the adjacent vessels and the sellar area. The 3D images of SCTA and simulated approach of operation supplied important information to the therapy planning. Conclusion The 3D images of SCTA , as an important supplement to conventional 2D images,can provide 3D images of neoplasms and clearly show the relationship among the neoplasms,adjacent vessels and the bones in the sellar area . This technique has a great value in the treatment of neoplasms in the sellar area.
