儿童神经母细胞瘤的影像学诊断

摘　　要：目的　了解儿童神经母细胞瘤的临床表现和发生及转移的情况 ,描述该病的影像学表现 ,以提高对本病的认识。方法　回顾性分析 2 8例经病理活检证实的儿童神经母细胞瘤的临床资料及影像学表现。结果　 2 8例中 ,原发部位为纵隔 8例 ,腹部 16例 ,颈部 1例 ,鼻咽部 1例 ,盆腔 1例。 1例未发现原发病灶而仅表现为头皮和颅内的转移。腹部肿瘤有8例 (8/16)转移 ,纵隔肿瘤有 4例 (4 /8)转移 ,盆腔 1例转移。 2 8例中共有 13例骨髓转移。结论　神经母细胞瘤可以发生在肾上腺髓质及交感神经链的任何部位 ,可伴有早期转移。影像学检查能全面地对病灶进行定位 ,并对临床分期及预后有一定帮助。Objective To analyze the clinical presentation,imaging manifestation,location of primary and metastatic neuroblastoma. Methods Twenty eight patients with neuroblastoma diagnosed by imagings,pathology and clinics were retrospectively reviewed. Results The primary neuroblastoma was found in the mediastinum (8),the retroperitoneum (16),the neck (1),the pharynasal region (1) and the pelvis (1).One case presented metastasis in the orbit and intra-cranium without detection of primary lesion. Conclusion The neuroblastoma could locate in the medulla of the adrenal gland and any part of sympathetic chain.The metastasis of neuroblastoma may occur in the early stage.The imaging modality is very helpful in detecting,staging neuroblastom and predicting its prognosis.
