糖尿病视网膜病变患者瘦素水平的变化

摘　　要：目的 :探讨糖尿病视网膜病变患者血清瘦素水平变化规律。方法 :受试对象 12 0例 ,分为对照组 30例 (男 18,女 12 )、糖尿病无视网膜病变组 30例 (男 18,女 12 ) ,非增殖性视网膜病变组 30例 (男 18,女 12 )及增殖性视网膜病变组 30例 (男 18,女 12 ) ,检测血清瘦素、胰岛素、Ⅳ型胶原及血糖浓度 ,分析瘦素水平与其它指标的相关性。结果 :对照组、无视网膜病变组、NPDR组及PDR组血清瘦素浓度分别为 6 91± 1 87μg/L ,7 83± 2 11μg/L ,9 5 6± 2 4 3μg/L和 11 6 9± 2 5 7μg/L ,经协方差分析排除体重指数的影响 ,PDR组血清瘦素浓度高于NPDR组 (t=2 15 ,p<0 0 5 )、无视网膜病变组 (t=2 71,p <0 0 1)及对照组 (t =3 5 0 ,p <0 0 0 1) ;NPDR组高于无视网膜病变组 (t=2 2 3,p <0 0 5 )及对照组 (t=2 75 ,p<0 0 1)。血清瘦素浓度与体重指数、胰岛素呈正相关 (r=0 2 2 ,p <0 0 5 ;r=0 2 8,p<0 0 1)。结论 :糖尿病视网膜病变患者血清瘦素浓度升高 。Objective To explore the regulation of changes in serum leptin level in patients with diabetic retinopathy. Methods The 120 participating subjects were of four groups: healthy controls, diabetic patients without retinopathy, patients with NPDR and patients with PDR, each group consisted of 18 males and 12 females with comparable BMI. The levels of serum leptin, ⅣC, insulin and blood glucose of these patients were measured and the correlation between serum leptin level and other parameters was analysed. Results The level of serum leptin in controls, diabetic patients without retinopathy, patients with NPDR and patints with PDR were 6 91±1 87μg/L,7 83±2 11μg/L,9 56±2 43μg/L and 11 69±2 57μg/L respectively. The patients with PDR had higher serum leptin levels than patients with NPDR ( t=2 15,p <0 05), diabetic patients without retinopathy ( t=2 71,p <0 01), and controls ( t=3 50,p <0 001), the patients with NPDR had higher serum leptin levels than diabetic patients without retinopathy ( t=2 23,p <0 05) and controls ( t=2 75,p <0 01), while the difference in serum leptin was not significant between diabetic patients without retinopathy and controls. The serum level was positively correlated to BMI ( r=0 22,p <0 05) and FINS ( r=0 28,p <0 01). Conclusion Serum leptin level is elevated in patients with diabetic retinopathy and is positively correlated to the severity of the disease.
