胃癌患者外周血肿瘤细胞的检测

摘　　要：目的　探讨监测胃癌患者早期血行转移的敏感方法 ,以及检测外周血肿瘤细胞对预测胃癌患者的微小转移、评估预后、提供治疗参考的意义。方法　采用RT -PCR方法检测 3 0例胃癌患者、2 0例良性胃病患者、10例健康对照者外周血中CK 2 0mRNA。胃癌组按临床病理分为TNMⅠ～Ⅳ期 ,TNMⅠ期 3例 ,Ⅱ期 9例 ,Ⅲ期 12例 ,Ⅳ期 6例。按组织分化程度分为高、中、低分化 ,高分化 4例 ,中分化 8例 ,低分化 18例。结果　良性胃病患者及健康对照者CK 2 0mRNA均阴性。胃癌患者CK 2 0mRNA阳性率为 43 .3 % (13 /3 0 )。TNMⅠ～Ⅳ期CK 2 0mRNA阳性率分别为 0 (0 /3 )、11.1% (1/9)、5 0 .0 % (6/12 )、10 0 .0 % (6/6)。高分化胃癌CK 2 0mRNA阳性率为 0 (0 /4 ) ,中分化及低分化分别为 2 5 .0 % (2 /8)、61.1% (11/18)。CK 2 0mRNA阳性率与胃癌患者的临床病理分期、肿瘤的分化程度密切相关 (P <0 .0 5 )。结论　RT -PCR技术检测CK 2 0mRNA可作为监测胃癌早期血行转移的敏感方法 ,CK2 0mRNA可作为预测胃癌患者微小转移、评估预后。Objective To study the value of detecting gastric carcinoma cells in peripheral blood for monitoring metastasis and evaluating prognosis of stomach carcinoma.Methods The cell marker CK20 mRNA was detected by RT PCR in 30 patients with gastric carcinoma,20 cases with benign gastropathy and 10 normal controls.Results 30 patients with gastric carcinoma were classified into TNM Ⅰ～Ⅳstages.There were 3,9,12,6 cases in TNM Ⅰ～Ⅳ,respectively.The positive rate of CK20 mRNA was 0(0/3)in stage Ⅰ,11.1%(1/9) in stage Ⅱ,50.0%(6/12) in stage Ⅲ,and 100.0%(6/6) in stage Ⅳ.These patients were also devided into 3 groups by differential degree.The positive rate was 0(0/4) in high degree,25.0%(2/8) in moderate degree and 61.1%(11/18) in low degree.CK20 mRNA was negative in peripheral blood from those with benign gastropathy and the normal controls.Conclusion RT PCR is a sensitive method for detecting CK20 mRNA;CK20 mRNA is a good marker for detecting gastric carcinoma cells in peripheral blood;CK20 mRNA can be used to monitor blood metastasis of the tomor in an early stage;CK20 mRNA is helpful for evaluation of prognosis and therapy.
