脑膜瘤形状与其复发关系的探讨

摘　　要：目的根据临床放射学特点辨别易复发的脑膜瘤,以便在术前选择更适当的手术方式,减少肿瘤的复发。方法根据CT和(或)MRI表现,对206例脑膜瘤的肿瘤形状分类,了解肿瘤形状与复发的关系。结果“磨菇状”和“分叶状”脑膜瘤复发率明显高于″圆形″脑膜瘤,而且复发间隔时间也较“圆形”脑膜瘤短(P<0.01)。结论“磨菇状”和“分叶状”脑膜瘤的手术应尽可能扩大切除与肿瘤邻近的脑膜和颅骨,减少肿瘤复发机会。Objective To explore relationship between the meningiomas shapes with their recurrence.Method The clinical and image data of206patients with meningiomas who were followed up for at least5years after the operation were analysed retrospectively.Results Of 206cases of meningiomas,7had the shape of mushroom,45lobulated shape and154round.During following up,5cases recurred in7mushroom tumors,23in45lobulated ones and15in154round ones.The average time of the tumor recurrence was10months in the mushroom meningiomas,62months in the lobulated ones and89months in round ones.Conclusion Meningiomas with mushrooming or lobulated shapes,which recurred more easily than the ones with round shape,should be treated by excision of the wider adjacent dura mater and bone to decrease the chance of recurrence.[
