膀胱粘膜白斑15例诊治体会

摘　　要：目的 :探讨膀胱粘膜白斑的诊断和治疗效果。方法 :采用抗感染 ,经尿道电切加气化术 ,电切前在膀胱粘膜下注射 2 .5 % 5 氟尿嘧啶 (5 FU) 1～ 2ml,使粘膜与肌层分离 ,以及膀胱内药物灌注等治疗。结果 :分别经过 6～ 52个月随访 ,1 1例治愈 ,2例 3年后恶变 ,2例失访。结论 :膀胱镜检加活组织检查对膀胱粘膜白斑恶变的早期诊断具有重要意义 ,经尿道电切加气化术以及膀胱内药物灌注是治疗膀胱粘膜白斑和预防其恶变的有效方法。Purpose:To investigse the diagnosis and treatment of leukoplakia in bladder. Methods:Transurethral resection as well as gasification were performed in 15 cases of urinary bladder leukoplakia.Before the operation, 5 FU had been injected beneath the bladder mucosa to depart it from muscle, then instillation treatment was followed.Results:15 cases were followed up for 6-52 months and 2 cases were lost.Among 15 cases,11 cases were cured and 3 cases were observed to develop malignant lesions in bladder muosa following the transurethral surgeries. Conclusions:Cystoscopy and biopsy could play an importont role in early diagnosis of the malignant changes in bladder leukoplakia. Transurethral resection and gasification plus instillation treatment were effective methods to treat leukoplakia of the bladder, and these methods could also prevent it from malignant changes.
