摘  要:目的 探讨脑电生物反馈治疗对多动性障碍 (HD)患儿脑功能的改善作用。方法 应用AutogenicA62 0型脑电生物反馈仪对 2 2例 (6~ 12岁 )HD患儿进行 40次增强 16~ 2 0Hzβ波 ,抑制 4~ 8Hzθ波的脑电反馈治疗 ,治疗前后采用脑电反馈仪检测患儿脑电波θ/ β功率比值的变化 ,采用数字划消试验及视听注意分配测试检测注意稳定性及注意分配的变化 ,采用光信号的视觉记忆测试 ,了解短时记忆的差异 ,采用韦氏儿童智力量表 ,评价治疗前后智力水平及分测验量表分的变化。结果 EEG反馈治疗后患儿θ/ β值较前明显下降 (P <0 .0 5 ) ,注意力测试中数字划消速度加快 (P <0 .0 0 1) ,错误数减少 (P <0 .0 0 1) ,注意分配值Q值明显增加 (P<0 .0 5 ) ,视觉记忆测试中的错按数明显减少 (P <0 .0 1) ,智力测试中VIQ、PIQ和FIQ均较前改善 (P <0 .0 1,P <0 .0 0 1) ,VIQ与PIQ的提高幅度之间比较无显著性差异 (P >0 .0 5 ) ,其中背数分测验量表分及由算术、译码、常识、背数 4项分测验组成的ACID模式量表分均较前明显提高 (P <0 .0 5 ,P <0 .0 1)。结论 脑电反馈治疗能够明显改善HD患儿的脑电异常 ,智力、注意力和短时记忆缺陷 。Objective To investigate the effect of EEG biofeedback treatment on improving brain function of children with hyperkinetic disorder(HD).Methods Twenty two children(ages 6~12)with HD diagnosed according to ICD 10 received 40 sessions of EEG biofeedback treatment. Feedback was contigent on enhancing 16~20 Hz beta activity and suppressing 4~8 Hz theta activity. All subjects were measured at pretreatment and at posttreatment on EEG activity,WISC R,Number Cancellation Test,Visual Memory Test and Attention Distribution Test.Results After treatment significant decreases were observed in the EEG ratio of the theta/beta(P<0.05),in the errors of Number Cancellation Test(P<0.001) and in the errors of Visual Memory Test(P<0.01). Significant inprovements were found in the speed of number cancellation (P<0.001 )and in divided attention(P<0.05).VIQ,PIQ and FIQ all increased markedly(P<0.01 and P<0.001 respectively). There was no significant difference between the increment of VIQ and that of PIQ(P>0.05). Significant increases were obtained in the scores of digit span and ACID pattern (P<0.05 and P< 0.01 respectively).Conlusions EEG biofeeedback is highly effective in improving EEG spectrum,intelligence,attention and short term memory of children with HD. The effect is not hemispheric specific.
