纤维蛋白胶在乳腺癌根治术中的作用

摘　　要：目的 :探讨纤维蛋白胶在乳腺癌根治术中的应用价值。方法 :将乳腺癌患者 4 3例 ,随机分为两组。一组 (2 3例 )行常规乳腺癌根治术 ,另一组 (2 0例 )行常规乳腺癌根治术同时在手术创面应用纤维蛋白胶 ,通过观察术后第 1天负压吸引的引流量及术后总引流量、皮下积液情况来确定纤维蛋白胶在乳腺癌根治术中的有效性。结果 :应用纤维蛋白胶组术后第 1天引流量及术后总引流量分别为 (4 5 .5± 2 1.3) ml和 (2 2 4 .8± 87.3) ml,对照组术后第 1天引流量及术后总引流量分别为 (10 2 .6± 4 3.7) ml和 (4 83.2± 14 3.5 ) ml,具有非常显著性差异 (P<0 .0 1)。应用纤维蛋白胶组术后皮下积液有 2例 ,而对照组有 10例 ,两组间也具有显著性差异 (P<0 .0 5 )。结论 :乳腺癌根治术中应用纤维蛋白胶可以有效减少术后引流量 ,降低皮下积液发生率 。Objective:To evaluate the efficacy of fibrin glue in radical mastectomy Methods:A prospective randomized trial was conducted consisting of 43 women All women received radical mastectomy At the completion of the mastectomy they were randomized to receive either standard closure or the application of fibrin glue before standard closure A total of 20 women received fibrin glue and 23 received no glue Patients were then monitored for the first day drain output postoperative,total drain output and seroma formation Results:The first day drain output postoperative and total drain output in the fibrin glue group averaged 45 5±21 3ml and 224 8±87 3ml compared to 102 6±43 7ml and 483 2±143 5ml in the control group( P <0 l01) Seroma in the fibrin glue group were two and ten in the control group( P <0 05) Conclusion:Application of fibrin glue can reduce the first day drain output postoperative,total drain output and seroma formation in patients undergoing radical mastectomy
