摘 要:目的:探讨精神分裂症和抑郁症脑结构改变特点。方法:研究51例首发精神分裂症、32例抑郁症和23例健康志愿者。以PANSS和HAMD减分率评定疗效。MRI测量第三脑室横径在内的17条径线。结果:病例组第三脑室横径、双侧外侧裂宽度显著增宽,胼胝体厚度显著减少。脑结构异常与疗效之间缺乏普遍相关。结论:脑结构异常可能构成了疾病的神经生物学基础,它们与疗效的关系尚有待进一步研究。AIM:To explore the structural brain abnormalities in depression and schizophrenia.METHODS :Quantitative MRI methods were used to address the regional specificity of structural brain abnormalities in 51first-epi sode patients with CCMD-2-R schizophrenia(mean age,28.02years)and 32patients with depression(meanage,39.50years)relative to23healthy controlsubje cts(mean age,35.43years).Allpatients were assessedwithPAN SSandHAMDatbaseline and 2months after treatment.Treatm ent response was evaluated accordin g to the decrease of PANSS and HAMD scores.RESULTS:Relative to healthy control subjects,bothschizophrenic and depressive patients group hada thirdv entricular enlargement,smaller temporallobe(leftandright ),anddecreaseddepthofcorpus callosum.No general correlation wa s found among structural brain abnormalities and clinical efficacy.CONCLUSION:These findings support the view that schizophrenia and depression may in volve disturbance of neurodevelopm ental processes in some patients.However,the correlation between structura l brain abnormalities and clinical efficacy remains uncertain.
