摘　　要：目的　探讨角膜移植术后肿瘤坏死因子 -α( tumor necrosis factor-α,TNF-α)含量变化与排斥反应的关系 ,寻找一种预测排斥反应的新方法。方法　分别以鸡、兔为供受体建立异种异体穿透性角膜移植模型 (鸡→兔 ) ,术后不同时间抽取兔房水及耳缘动脉血 ,运用酶联免疫吸附 ( enzyme linked im munosobent assay,EL ISA)双抗体夹心法检测 TNF- α含量的动态变化。结果　异种异体角膜移植 ,植片平均存活 18.8d± 3.0 d。正常兔房水及外周血中 TNF- α含量分别为 ( 31.2 5± 17.13) ng· L- 1 、( 6 9.98± 17.40 ) ng· L- 1 。角膜移植术后 3d,各组房水及外周血中 TNF-α含量即升高 ,其后自体移植组含量下降并在 2周内降至正常水平 ,但异种异体移植组继续升高 ,第 2周时达最高水平 ,并在整个观察期内维持高水平。相关分析显示血、房水 TNF-α含量变化正相关 ( r=0 .94,P<0 .0 1)。结论　角膜移植术后外周血 TNF- α含量变化可反应局部免疫学状态 ,监测外周血 TNF- α含量变化可预测排斥反应的发生。ObjectiveTo investigate the relationship between the tumor necrosis factor-α(TNF-α) content and corneal transplantation immunologic rejection. MethodsThe chicken to rabbit model of penetrating heterograft was used. The levels of TNF-α in serum and aqueous humor were determined dynamically by enzyme linked immunosobent assay (ELISA) in different postoperative days. ResultsThe grafts of heterograft group survive (18.8±3.0)days. The mean aqueous humor and serum levels of TNF-α in normal animals were (31.25±17.13)ng·L -1?(69.98±17.40)ng·L -1 respectively. In all operative groups the levels increased in early postoperative days. Then, the levels decreased and gradually reached the normal in autograft group. However the levels in heterograft group continued increasing and reached the highest levels in the second week after operation, and during the period of observation the level kept high. Correlation analysis showed the change of TNF-α content in aqueous humor and serum was positive correlation (r=0 94, P<0.01). ConclusionThe TNF-α levels in serum showed the local immune situation. Sequential determinations of the TNF-α content in serum after operation may be valuable in prediction and diagnosis of rejection.
