弥漫型胃癌48例临床病理分析

摘　　要：目的　探讨弥漫型胃癌的临床、病理特点。方法　对 1 995- 0 1～ 2 0 0 2 - 0 5经手术切除治疗 ,病理检查诊断的1 4 9例胃癌按 Lauren分型 ,其中弥漫型胃癌 48例并进行回顾分析。结果　本组胃癌中男 1 9例 ,女 2 9例 ,男女之比为1∶ 2 .52 ,女性多于男性。大体形态多为 Borrmann 、 、 ,占 97.7%。组织学形态表现为低分化的单核样癌细胞及印戒细胞浸润伴组织细胞、嗜酸性粒细胞、淋巴细胞、浆细胞浸润 ,癌细胞浸润粘膜下及肌间隙使其增宽 ,癌细胞粘液染色阳性。结论　弥漫型胃癌占 L auren分型的 32 .2 % ,恶性度高 ,确诊时多为晚期 ,多发于青年女性 ,其次 55岁以上老年男性也多见。Objective To investigate the clinical and pathological features of diffused gastric carcinoma.Methods 48 specimens of diffused gastric cancers classified by Lauren classification from 149 specimens of gastric cancers diagnosed by pathology were analyzed retrospectively.Results There were 19 males and 29 famales ( 1∶2.52).Most of their pathomorphologic classifications were Borrmann Ⅱ?Ⅲ?Ⅳ(97.7%).Histologically there were poor differentiated mononuclear cancer cell and signet ring cell infiltration accompanied by histocyte,eosinophilic granulocyte,lymphocyte,plasmosyte infiltration,submucouslayer and intermuscular infiltrations by cancer cells.Cancer cells had positive mucoid stain.Couclusions 32.2% of lauren classification were diffused gastric cancer.It was very malignant.Most comfirmed diagnosis were in late stage.It occured mostly in young women,followed by the old men more than 55.It's histopathologic diagnosis should be distinguished from that of gastric eosinophilic granuloma.<
