角膜穿通伤合并外伤性白内障疗效分析

摘　　要：目的:总结45例45眼复杂多样的角膜穿通伤合并外伤性白内障病例,对于采用不同的方法所取得的疗效进行分析。方法:角膜穿通伤均在显微镜下予以清创缝合,并采用前房穿刺、注入粘弹剂等辅助措施。一期行超声乳化白内障摘除人工晶体植入术18例;外伤缝合后经抗炎治疗,稳定3个月后,再行二期超声乳化人工晶体植入术27例。结果:45例匀获得良好的疗效。外伤缝合均使创口达到水密状态、维持正常的前房深度及眼压。并成功地进行了白内障手术、植入人工晶体于后房,视力有不同程度的提高:0.1以下3例,0.1-0.3为19例,0.3-0.5为17例,0.5-0.8为5例,1.0为1例。结论:角膜穿通伤的缝合是本病治疗的基础;而外伤性白内障的摘除及人工晶体植入的时机和方式的选择,对病人术后视力的恢复程度至关重要。Objective: To analyze effect of treating 45 cases 45 eyes of corneal perforating injuries combined with traumatic cataract in different ways. Methods:AIl the corneal perforating injuries were sewed up under surgery microscope and assisted with anterior chamber puncture and Healon. Phacoemulsification and intraocular lens implantatoin was performed on 18 cases at the same time and 27 cases three months later after anti-inflammatory cure. Results: The 45 cases got satisfactory effect with the wound sealed up and IOL implantation. The visual acuity (VA) of the all 45 cases 45 eyes improved , VA below 0.1, 3 eyes, VA0. 1-0.3, 19 eyes, VA0.3 -0.5, 17 eyes,VA0.5 -0.8, 5 eyes, VA 1.0, 1 cases. Conclusion: The suture surgery of the corneal perforating injuries was the base, and choice of time and method of phaco and IOL plantation was very important for the recovery of sight.
