摘 要:目的 :探索近视散光眼的高阶像差与对比敏感度的关系 ,为屈光手术中像差引导的个性化切削的个体筛选提供客观依据。方法 :随机选择近视屈光手术术前接受检查的6 6例 132眼 ,平均年龄为 (2 3.16± 4 .87)岁 (19~ 4 1岁 ) ,球性屈光度为 (- 6 .82± 1.38)D(- 3.715D~ - 9.32 5D) ;柱性屈光度为 (- 0 .71± 0 .5 1)D(- 0 .0 8D~ - 2 .85 5D)。用主觉法测量屈光度数 ,并用CSV 10 0 0对比敏感度表灯测试最佳矫正视力下的暗室对比敏感度值 ,用AllegrettoWavefrontAnalyz er波前像差分析仪测量其像差值。结果 :①球 柱像差对 18c/d频段的对比敏感度有影响 (r=- 0 .196 ,P =0 .0 2 5 ) ;彗差没有表现出与对比敏感度有关的任何相关性 (P >0 .0 5 )。②近视度数和球 柱像差相关性为r =- 0 .5 0 2 (P =0 .0 0 0 ) ,和收稿日期 :2 0 0 2 -10 -2 5 ;修回日期 :2 0 0 2 -11-10作者简介 :江扬子 ( 1975 -) ,女 ,浙江温州人 ,医学硕士。通信作者 :王勤美 (E -mail:wqm @mail.eye .ac .cn)。彗差相关性为r=- 0 .2 81(P =0 .0 0 1) ,散光度数和球 柱像差相关性为r=- 0 .2 4 1(P =0 .0 0 5 ) ,和彗差相关性为r=-0 .176 (P =0 .0 4 3)。结论 :屈光手术中对高度数的近视散光以及高球 柱像差者进行波前像差引导的个性化?Objective:To investigate relationship between higher order aberrations and visual quality in myopic astigmatism eyes,offer clinic data in selecting persons who need wavefront guided custom ablation.Methods:66 subjects 132 myopic astigmatism eyes were observed,n=132,age(23.16±4.87)yr,range 19~41 yr;myopic diopter(-6.82±1.38)D,range -3.715~-9.325 D;astigmatic diopter(-0.71±0.51)D,range -0.08~-2.855 D. Manifest refraction were measured by phoropter;best spectacle corrective contrast sensitivity were measured by CSV 1000;aberrations were measured and analyzed by Allegretto Wavefront Analyzer.Results:①There is significant correlation between sphcl and high frequency(18 cyc/deg) contrast sensitivity(r=-0.196,P=0.025),coma has not showed any relationship with contrast sensitivity(P>0.05).②The analysis of factors associated with aberrations show myopia correlated significantly with sphcl and coma(r=-0.502,P=0.000;r=-0.281,P=0.001);astigmatism also display obvious relationship with sphcl and coma(r=-0.241,P=0.005;r=-0.176,P=0.043).Conclusion:Higher myopic astigmatism will affect sphcl aberration,high sphcl aberration influent high frequency visual quality,it indicates that high myopia,high astigmatism and high sphcl aberration will benefit from wavefront guided coustom ablation.
