饮水负荷试验测定非溃疡性消化不良患者胃感觉阈值

摘　　要：目的 了解非溃疡性消化不良(NUD)患者的感觉阈值。方法　运用饮水负荷试验对50例NUD患者和15例正常健康人胃感觉阈值进行测定,在4min的饮水过程中,初次出现上腹部有感觉(阻塞、膨胀等)时的饮水量表示胃感知阈值;当受试者出现明显持续无痛性中度上腹饱胀等不适时的饮水量表示胃不适阈值。结果　NUD病人胃感知阈值和胃不适阈值均较正常健康人显著降低(426.7±106.5 ml比530.0±86.2ml,P<0.05和781.7±131.6 ml比930.0±127.9 ml,P<0.05);38％(19/50)的 NUD患者胃感知感觉过敏,36％(18/50)的NUD患者胃不适阈值低于正常下限值。结论 饮水负荷试验证实部分NUD病人存在胃感觉过敏,参与NUD患者的发病。Objective To study gastric sensation threshold in patients of non - ulcer dys-pepsia(NUD) .Methods The gastric sensation threshold were monitored in 50 NUD patients and 15 healthy people by drinking water load test. The volume of drinking water was regarded as sensation threshold when subjects reported the initial perception epigastric filling and bloating during water drinking. The volume was discomfort threshold when subjects reported constant non -painful and moderate epigastric discomfort or bloating. Results NUD patients had significantly lower gastric sensation threshold for both perception and discomfort evoked by drinking water(426. 7 ±106. 5 ml v. s 530 ±86. 2 ml and 781. 7±131. 6 ml v. s 930.0 ±127.9 ml, P<0. 05 respectively). 38% NUD patients had hypersensitive gastric perception threshold. 36% NUD patients had hypersensitive gastric discomfort threshold. Conclusion Gastric hy-persensitivity which exists in NUD patients plays an important role in the development of non -ulcer dyspepsia.
