腋淋巴结阴性乳腺癌中组织蛋白酶D表达意义的比较研究

摘　　要：目的 对组织蛋白酶D(Cath-D)在腋淋巴结阴性(ANN)乳腺癌中不同部位的表达与预后的相关性进行比较性研究。方法 根据术后病理检查腋淋巴结有否转移及根据随访结果选取192例乳腺癌病例作为本项目研究对象,并将其分为ANN预后差组、ANN预后好组和淋巴结阳性(ANP)组3组。采用免疫组化S-P法检测全部病例中Cath-D的表达,根据不同的表达部位,采用三种不同的评价方法分析比较,并用χ2。检验、χ2分割法和u检验进行统计学处理。结果 Cath-D阳性表达为棕黄色着色,定位于部分癌细胞和间质细胞胞质中;ANN预后差组和ANP组癌细胞Cath-D阳性率明显高于预后好组(χ2=23.20,P<0.01);ANP组表达与预后差组间无统计学差异(χ2=0.19,P>0.05);三组间间质细胞Cath-D表达阳性率无统计学差异(χ2=1.56,P>0.05);癌细胞和间质细胞表达均计入阳性率时,其阳性率与仅观察癌细胞表达无显著意义的提高(u1=0.47;u2=1.41;P>0.05)。结论 Cath-D可以作为ANN乳腺癌预后差的标志物之一,仅观察评价其在癌细胞中的表达是比较可靠、实用和简便的方法。Objective To investigate the relation between prognosis and the Cathepsin-D(Cath-D) expression in neoplastic and stromal cells of the axillary lymph nodes negative (ANN) breast cancer.Methods 192 cases with breast cancer were selected according to the axillary lymph node situation and the results of the follow-ing-up. These cases were divided into 3 groups, ANN breast cancer with poor prognosis group, ANN breast cancer with good prognosis group and axillary nodes positive (ANP) group. Cath-D expression were detected by immuno-histochemistry (S-P method) . Depending on expression of Cath-D in different cells, three different evaluative methods were used and the statistical analysis (x2 test,x2 cutting apart method and u test ) were performed.Results The positive expression of Cath-D was defined as brown-yellow stain which located in the cell serum of neoplastic and stromal cells. The positive rate of Cath-D expression in ANN breast cancer with poor prognosis group and axillary nodes positive (ANP) group were definite significant higher than that of ANN breast cancer with good prognosis group(X2 = 23.20,P<0.01). While expression of Cath-D in ANP group and in ANN with poor prognosis group had no significant difference (X2 = 0.19,P>0.05);Cath-D expression in stromal cells had no statistics difference among three groups(X2 = 1.56, P > 0.05) ;The aggregate rate of expression in neoplastic and stromal cells is not significant higher than that only in neoplastic cells( u, = 0.47; u2 = 1.41; P > 0.05). Conclusion These results suggest that Cath-D expression is one of the powerful prognostic marker in ANN breast cancer. It's a more reliable, practical,and convenient method only to observe and evaluate Cath-D expression in neoplastic cells.
