摘  要:目的 检测口腔鳞癌患者化疗前后MDR1、LRP、MRP、TopoⅡα的mRNA的表达情况,分析其与临床病理参数及化疗敏感性的关系。方法 采用RT-PCR,对22例口腔鳞癌患者术前诱导化疗前后MDR1、LRP、MRP、TopoⅡ α的mRNA表达进行测定。化疗方案为:平阳霉素16mg/m2,第1~10天,休息2周后行手术治疗。结果 (1)在口腔鳞癌组织中,同时存在多种MDR基因mRNA超表达的现象,其中MDR1 mRNA的平均表达率为(0.86±0.95),LRP mRNA的平均表达率为(1.57±0.81),MRPmRNA的平均表达率为(1.26±0.69);且MDR1 mRNA在癌组织的表达水平明显高于癌旁组织(P<0.05),表明口腔鳞癌多药耐药性与内在性耐药有关。(2)MDR1、LRP、MRP及Topo Ⅱα的mRNA表达水平与临床分期、肿瘤细胞的分化程度无明显相关性。(3)化疗有效者MDR1 mRNA表达水平明显低于无效者(P<0.05),而LRP、MRP及TopoⅡα的mRNA表达在二者差异无显著性。结论 口腔鳞癌的多药耐药性涉及到MDR1、LRP、MRP等多种基因的表达,但只有MDR1的mRNA表达与平阳霉素化疗反应性呈负相关。Objective The aim of this study was to examine the expression of mRNA of multidrug resistance gene (MDRl) ,lung resistance-related protein (LRP) ,multidrug resistance-associated protein (MRP) , topoi-somerase II a(Topo II a) before and after pre-operative chemotherapy in patients with oral squamous cell carcinoma (OSCC),and then to evaluate their roles in relation to clincopathologic factors and response to chemotherapy. Methods 22 cases with OSCC were given one cycle chemotherapy before operation. Chemotherapy regimen consisted of single Pingyangmycin agent. The expression of MDRl mRNA, LRP mRNA, MRP mRNA, Topo II a mRNA were examed with reverse-transcription polymerase chain reaction (RT-PCR) . Results The mean expression levels of mRNA of MDR-related markers in OSCC were 0.86 ± 0.95 for MDRl mRNA, 1.57 ±0.81 for LRP mRNA, 1.26 ± 0.69 for MRP mRNA respectively. The expression levels of mRNA of MDR-related markers in cancer tissues were higher than those in tumor adjacent oral mucosa( P < 0.05) . The results above suggested that the multi-drug resistance of OSCC might be derived from the intrinsic multidrug resistance. There was no significant relationship between the expression of the all the tested markers and the clinical stages, differentiation of the tumor. The expression level of MDRl mRNA in responders was lower than that in non-responders( P <0.05) ,and those of other MDR-related markers were no significantly difference between the responders and non-responders. Conclusion The multidrug resistance in patients with oral squamous cell carcinoma was related to the expression of MDRl, LRP and MRP genes. Only the MDRl mRNA expression was significantly associated with response to treatment.
