摘　　要：目的　探讨CK 19mRNA检测对判断乳腺癌微转移的价值。方法　应用RT -PCR法检测 66例乳腺癌患者术前和综合治疗 6个月、1年后外周血中CK 19mRNA表达状况 ,并以 3 0例乳腺良性疾病患者和 3 0例正常健康女性作为对照。结果66例乳腺癌和 3 0例乳腺良性疾病患者CK 19mRNA的阳性率分别为 90 .9% (60 /66)和 3 .3 % (1/3 0 ) ,3 0正常健康女性均阴性 ,相互比较有非常显著性差异 (P <0 .0 1)。有淋巴结转移者和无淋巴结转移者阳性率分别为 97.7% (4 3 /4 4)和 77.3 % (17/2 2 ) ,相互比较有显著性差异 (P <0 .0 5 )。综合治疗 6个月和 1年后阳性率分别为 2 8.8% (19/66)和 3 3 .3 % (2 2 /66) ,与术前 (90 .9% ,60 /66)比较有非常显著性差异 (P <0 .0 1)。结论　CKObjective To explore the value of CK19 mRNA in predicting of micrometastasis of breast cancer.Methods The CK19 mRNA in the peripheral blood of 66 cases with breast cancers was tested with RT PCR method postoperatively,six months,and one year after receiving comprehensive treatment,30 patients with benign breast disease and 30 healthy women were taken as control.Results The positive rates of CK19 mRNA in patients with breast cancers and benign breast diseases were 90.9%(60/66) and 3.3%(1/30) respectively.CK19 mRNA was all negative in the 30 healthy women.The difference of the positive rate between the group with breast cancer and the control group was significant( P <0.01).The positive rates of CK19 mRNA in patients with and without lymph nod metastasis were 97.7%(43/44) and 77.3%(17/22) respectively( P <0.05).And after receiving comprehensive treatment for six months and one year,the positive rates were reduced to 28.8%(19/66) and 33.3%(22/66) separately.The positive rates of CK19 mRNA is significantly different( P <0.01) between the results after operation(90.9%) and after comprehensive teatment.Conclusion The CK19 mRNA examined with RT PCR method can be applied to predict the micrometastasis of breast cancer in early phase.
