胃癌患者外周血中CEA mRNA的检测及其临床意义

摘　　要：目的　探讨胃癌患者外周血中CEAmRNA表达的临床意义。方法　采用巢式RT -PCR法 ,检测 42例胃癌患者外周血中的CEAmRNA表达情况。采用化学发光法检测血清中CEA水平。以胃腺癌细胞株SGC -790 1作为阳性对照 ,以 2 0例正常人外周血作为阴性对照。结果　 42例胃癌患者CEAmRNA阳性率为 45 .2 % (19/ 42 ) ,其中 2 5例行根治术的胃癌患者肿瘤切除术后血CEAmRNA阳性率明显高于术前 (5 6.0 %V .s 2 8.0 % ,P =0 .0 3 9)。胃癌患者外周血中CEAmRNA表达与肝转移相关 ,但与血清CEA值、肿瘤部位、UICC分期及分化程度无关。胃腺癌细胞株SGC -790 1中CEAmRNA均呈阳性表达 ;2 0例正常人外周血中CEAmRNA均呈阴性表达。结论　胃癌患者外周血中CEAmRNA表达可作为癌细胞血道播散的标志 ,对判断其预后及指导临床治疗具有重要意义。胃癌手术中应采用“不接触技术”Objective To evaluate the clinical significance of the expression of Carcinoembryonic Antigen mRNA(CEA mRNA) in peripheral blood of patients with gastric cancer.Methods Nested RT PCR technique was used to detect CEA mRNA in peripheral blood of 42 patients with gastric cancers before and after tumor resection.The serum CEA was determined by Chemiluminescence reaction.The SGC 7901 gastric cancer cell line and peripheral blood cells from the healthy people were used as positive and negative control,respectively.Results The positive rate of CEA mRNA in the patients with gastric cancers was higher after tumor resection than that before operation(56.0% V.s 28.0%, P =0.039).The expression of CEA mRNA in peripheral blood was significantly correlated with liver metastasis.However,there was no significant correlation between CEA mRNA and serum CEA level or other clinicopathologic characteristics.CEA mRNA was positive in all SGC 7901 cell line,but negative in peripheral blood of all healthy poeple.Conclusion The presence of CEA mRNA in peripheral blood is a helpful indicator of the hematogenic dissemination of tumor cells in patients with gastric cancer.It can be helpful to the judgement of prognosis and selection of treatment methods.Surgical manipulation may reduce the hematogenic dissemination of cancer cells.Furthermore,“no touch isolation” technique should be reinforced during operation.
