幽门螺杆菌与胃角溃疡边缘细胞增殖的关系

摘　　要：目的 :通过研究Hp相关溃疡边缘组织病理变化及增殖细胞核抗原 (PCNA)阳性表达 ,探讨HP与胃溃疡边缘细胞增殖的关系 ,初步了解HP致胃癌的可能途径。方法 :SP免疫组织化学方法检测溃疡边缘PCNA阳性表达情况 ,HE染色观察病理改变 ,粘膜涂片Giemsa染色及RUT确定HP感染。结果 :HP相关GU边缘组织表现为CSG、CAG、IM、DYS、GC ;GU边缘组织PCNALI %随着病变发展而升高 ,自CSG的 (2 3.93±14 .30 ) %升至癌变时的 79.8% ;HP相关GU边缘组织PCNALI%明显高于HP阴性GU(P <0 .0 5 )。结论 :HP感染导致炎症 ,HP与炎症及其所致的病理改变 ,使细胞增殖不断上调 ,超常的细胞增殖导致癌变的发生。其途径可能为HP→CG→CAG→IN→DYS→GC。HP在胃癌的发生发展过程中不仅起始动因子的作用 ,并可能参与胃癌发生的全过程。Objective:Through the research of border tissue of Helicobacter pylori (Hp)-relative gastric ulcer and positive expression of proliferating cell nuclear antigen(PCNA), to discuss the relationship of the Hp and cell proliferation of gastric ulcer, and to study possible process of gastric cancer induced by Hp. Methods: SP immunohistochemical method was used to study the PCNA positive expression of border tissue of ucler; Roution pathological section stained with HE was used to study the pathological changes of border tissue of ulcer: HP infection was determined by Giemsa stain of mucous membrane slice spreaded and the rapid urease test (RUT).Result:The border tissue of Helicobacter pylori (Hp)-relative gastric ulcer manifest CSG, CAG, IM, DYS, GC; Following the advance of pathological process, PCNALI% of border region of GU increase continously,from the(23.93±14.30)% of superficial gastritis to 79.8% of canceration.PCNALI% of border region of HP-positive GU is distinctly higher than the HP-negative ucler ( P <0.05).Conclusion: HP infection can cause inflammation,Hp?inflamma-tion and pathological changes cause the continous inetreasement of cell proliferat ion, unusual proliferation causescanceration. The possible way is: HP→CG→CAG→IM→DYS→GC. Hp not only acts as the starting factor during the occurrence and development of GC,but also join in the whole process of canceration.
