胃癌标本的X线形态与bcl-2蛋白表达的关系

摘　　要：目的　探讨bcl 2蛋白与胃癌标本X线形态学表现及临床病理因素之间的关系。方法　经低张气钡上消化道钡剂造影检查 ,确诊胃癌患者 3 5例 ,手术切除后新鲜标本沿大弯 (或小弯 )剖开 ,摄取大体标本照片 ,标本均匀涂钡 ,摄取标本涂钡照片 ;采用免疫组化SP方法检测 3 5例手术切除胃癌标本中bcl 2蛋白的表达。结果　胃癌组织中bcl 2阳性表达率为 82 86% ,明显高于癌旁及正常胃粘膜组织 (P <0 0 1) ,且与胃癌的分化程度及淋巴结转移有关 (P <0 0 5 )。 3 5例胃癌标本涂钡照片 ,其中 2 9例清晰显示癌肿周围粘膜破坏情况 ,在高中分化胃癌中 ,癌肿组织与周围正常粘膜皱襞中断截然的为 42 86% ,而在低分化胃癌中 ,80 %的患者 ,癌肿与周围正常粘膜皱襞中断截然 ,但经统计学处理P =0 0 6,无统计学意义 ;在胃癌的大体分型、淋巴结转移及浸润深度方面 ,与粘膜皱襞中断形式无统计学意义。结论　bcl 2在胃癌发生、发展过程中起重要作用 ,可作为胃癌预后的指标 ,但胃癌涂钡标本中粘膜皱襞中断形式与bcl 2基因之间的关系 。Objective: To investigate the correlation between the expression of bcl-2, X-ray morphology,and the pathologic factors in gastric cancer. Methods: The confirmed 35 cases of gastric cancer were examined by the lower tonic air-barium double contrast.After the operation,the specimens were photographed by Olympus digital camera and then recorded with X-ray film after evenly coated with barium in the surface of membrane at HITACHI Tu-21 X-ray Machine. The expression of bcl-2 was studied in the 35 cases of gastric cancer by SP immunohistochemical technique. Results: In all the cases examined,82.86% showed positive bcl-2 expression. There was a significant difference between the expression of bcl-2 in gastric cancer and paracancerous and normal tissues(P<0.01).Bcl-2 expression was positively correlated with the grade of differentiation and lymph node metastasis. The analysis of twenty-nine cases indicatd the abruption of mucosal folds,and there was complete abruption between the carcinoma and the normal tissues in poorly differentiated type; while in well/moderately differentiated type,there was obscure abruption,but no statistical correlations. No significant correlations were found between abruption of mucosal folds, lymph node metastasis,and the grade of differentiation. Conclusion: Bcl-2 plays an important role in the development of gastric cancer and bcl-2 is a fairly good index to indicate prognosis of gastric carcinoma,but further research must be done to reveal the correlation between the abruption of mucosal folds and the expression of bcl-2 in gastric carcinoma.
