摘 要:目的 探讨急性脑卒中患者内皮素 (ET)和神经元特异性烯醇化酶 (NSE)含量变化的临床意义和机制。方法 应用放免法测定急性脑卒中患者血液和脑脊液中ET及NSE的含量 ,并进行不同病变面积、不同病情、不同病程的ET及NSE含量的相关性分析。结果 急性脑出血和脑梗死组患者血、脑脊液中ET及NSE的含量均明显高于正常对照组 (均P <0 .0 0 1) ;且两组ET和NSE值与病情轻重、病灶面积大小有关 ;它们在卒中后 2 4小时即升高 ,3~ 7天达高峰 ,14天开始降低。结论 脑卒中急性期血和脑脊液ET及NSE含量呈正相关 (P <0 .0 5 ) ,说明ET和NSE一样可作为神经元损伤的定量生化标志物 ,其水平高低能反映脑卒中后脑组织损伤的严重程度以及神经功能的损害程度 ,可作为脑卒中患者病情监测和预后评估的指标之一。Objective To study mechanism and clinical meaning of endothelin(ET) and neuron specific enolase (NSE) in patients with acute stroke.Methods The contents of ET and NSE in blood and cerebrospinal fluid were measured by RIA in patients with acute stroke, and correlation analysis of the contents of ET and NSE with different lesion area,severe degree and different period of the disease was performed.Results The contents of ET and NSE in acute cerebral hemorrhage and cerebral infarction groups were significantly higher than those in control group ( P <0.001), and their levels in two groups were related to degree and area of the hemorrhage and infarction. They increased at 24 hours after stroke, and reached maximum within 3~7 days, and decreased after 14 days.Conclusion The contents of ET and NSE in blood and cerebrospinal fluid in patients with acute stroke were positively correlation. It indicates ET and NSE may serve as quantitative biochemical markers, and their levels may reflex the degree of brain damage. Therefor, they may serve as indexes of monitor and prognosis in patients with stroke.
