支气管动脉化疗与化疗栓塞治疗中晚期肺癌(附214例报告)

摘　　要：目的　分析介入化疗及化疗栓塞对中晚期肺癌的疗效。方法　中晚期肺癌 2 1 4例 ,均经病理证实。先行支气管动脉造影确认为肿瘤供血动脉后 ,A组经导管注入化疗药物 ,B组在灌注化疗后经导管注入 5 -Fu载药微囊 ,C组在灌注化疗后经导管注入明胶海绵微粒。结果　A组有效率为 63 .8% ,B组有效率为 88.7% ,C组有效率为 82 .1 % ,3组间治疗效果有显著差异 (χ2 =1 3 .73 =2 ,P <0 .0 1 ) ,经 χ2 分割B组与C组无显著性差异 (χ2 =0 .57, =1 ,P >0 .449) ,A组与B组、C组间有显著性差异 (χ2 =1 3 .1 2 , =1 ,P <0 .0 0 1 )。Objective To analyze the treatment effects on intermediate or advanced bronchogenic carcinoma by means of bronchial arterial infusion (BAI) or bronchial arterial embolization (BAE) .Methods There were 214 pathologgically proved cases of bronchogenic carcinoma in mid or late period.The patients were divided into BAI group,microsphere-entrapped 5-Fu (Ms-5-Fu) group,gelform group.BAI group included 105 cases,Ms-5-Fu group included 53 cases and gelform group included 56 cases.After detecting the nourishing arteries of bronchogenic carcinoma with angiography ,anticancer drugs,anticancer drugs and Ms-5-Fu,anticancer drugs and gelfrom were infused from nourishing arteries,in three groups of patienyts respectly.The effective rates of the treatment were compared in patients among three groups.Results The effective rate of BAI group was 63.8%,that of Ms-5-Fu group was 88.7% and gelform group was 82.1%.The effective rates of treatment were obvious difference (χ 2=13.73,=2,Ρ<0.01).There was no significant statistic meaning though chi-squre test between Ms-5-Fu group and gelform group(χ 2=0.57,=1,Ρ>0.449).There was significant statistic meaning between BAI group and Ms-5-Fugroup add gelform group(χ 2=13.12,=1,Ρ<0.001). Conclusion BAE is more effective than BAI in treating intermediate or advanced bronchogenic carcinoma.
