后巩膜加固术治疗进行性近视的临床研究

摘　　要：目的 :研究后巩膜加固术 (PSRD)治疗进行性近视的疗效及机理。方法 :对 2 0例 ( 3 6眼 )进行性近视行改良的Snyder Thompson式PSRO ,手术前后观察裸眼视力、矫正视力、眼底、屈光度、眼轴、闪光视网膜电图 (F -ERG) ,振荡电位 (Ops)。术后随访 1年者 3 6眼 ,5年以上 13眼。结果 :( 1)术后 90 %以上的高度近视患者视力稳定 ,部分患者视力有不同程度的提高。 ( 2 )术后眼轴及屈光度稳定 (P >0 0 5 )。( 3 )F -ERG的a波b波波幅术后较术前有提高 (P <0 0 5 ) ,Ops振幅有非常显著性提高 (P <0 0 0 2 ) ,且均随时间的延长 ,提高更加明显。结论 :后巩膜加固术治疗进行性近视 ,的确起到稳定眼轴及屈光度 ,改善黄斑病变及视网膜血液循环 ,从而提高视功能的作用。Objective:The observe the therapeutic effects of posterior scleral reinforcement operation (PSRO) in the treatment of progressive myopia and to study the mechanism. Methods:20 cases (36 eyes) of progressive myopia underwent PSRO in Snyder-Thompson′ modification.13 eyes were observed 5 years after surgery,and the rest for 1 year. Results:Postoperative visual acuity was stable in above 90% of the eyes and most of them were better than preoperation.The ocular axial length and refractive degree were stable after operation.There were significant increased amplitudes of a,b waves of F-ERG and Ops waves.The longer the follow up period,the more significant the increased amplitudes of Ops waves. Conclusions:PSRO in the treatment of progressive myopia plays a role of stabilizing ocular axial length and refractive degree,improving the macular status of high myopia and the blood circulation of the macular area.
