摘  要:目的 探讨预先的TIA是否对后继脑梗死有保护作用。方法 对 6 6 3例脑梗死患者 ,按照是否预先发生同侧的TIA及其持续时间分为TIA短于 2 0min组 ,TIA 2 0~ 6 0min组和无TIA组 ,并于病程 1个月时对各组进行生活能力评分 ,评定预后 ,分析预后与TIA持续时间、发作次数及TIA与脑梗死间隔时间的关系。结果 脑梗死前有TIA发作短于 2 0min、发作 2~ 3次与脑梗死间隔不超过 1周者 ,其预后明显好于无TIA者 (P <0 .0 1或 0 .0 5 )。结论 脑梗死前发生的同侧TIA可能产生缺血耐受现象 ,对后继发生的脑梗死起保护作用。Objective To study whether prior ipsilateral TIA has a neuroprotective effect on subsequent ischemic infarct.Methods 663 patients of cerebral infarct (CI) were divided into three groups on the basis of the presence or absence of prior ipsilateral TIA and the duration of TIA (TIA< 20 minutes, TIA 20~60 minutes, No TIA).According to the scores of activities of daily living (ADL) at one month in three groups respectively, the outcomes were assessed.The relationship was analysed between the outcome and duration of TIA? the number of TIA and intervals of TIA and CI.Results The patients with previous TIA lasting less 20 minutes had better outcomes than those without TIA (P< 0.01).The frequency of TIA from 2 to 3 times and the intervals of 1 week or less between TIA and CI were still related to a favorable outcome (P< 0.05 respectively).Conclusions This study indicates that ischemic tolerance may play a role in patients with psilateral TIA before CI, inducing the neuroprotective effect for a subsequent ischemic stroke.
