摘　　要：目的 :探讨胃粘膜癌前病变与胃癌之间的关系。方法 :应用免疫组化SP法检测异型增生、肠上皮化生和萎缩性胃炎等胃癌前病变组织中细胞增殖核抗原 (PCNA)、表皮生长因子 (EGFR)、凋亡抑制基因 2 (bcl 2 )的表达 ,并与胃癌及正常胃组织进行对照比较。结果 :胃癌与异型增生比较 ,PCNA、EGFR、bcl 2的表达均无显著性差异 (P均 >0 .0 5 ) ;与肠上皮化生和萎缩性胃炎比较 ,PCNA、EGFR的表达有高度显著性差异 (P均 <0 .0 1) ,bcl 2的表达有显著性差异 (P <0 .0 5 ) ;与正常胃粘膜比较 ,PCNA、EGFR、bcl 2的表达均有高度显著性差异 (P均 <0 .0 1)。结论 :PCNA、EGFR、bcl 2的表达在胃粘膜的恶性转化过程中起着重要作用 ,联合检测这些指标有助于癌前病变和早期胃癌的诊断。Objective:To explore the relationship between the precancerous lesion and gastric cancer.Methods: S P immunohistochemistry was used to detect the PCNA,EGFR,bcl 2 in the specimen of atrophic gastritis,intestine epithelial metaplasia,hyperplasia and compared with normal gastric tissues.Results:There were significant differences in these indexes between intestine epithelaial metaplasia,hyperplasia and gastric cancer (P<0.01),but there were no differences between atrophic gastritis and gastric cancer.Conclusion:The expression of PCNA,EGFR and bcl 2 play an important role in the process of cancerous trasformation of gastric mucosa.Combining detection of these indexes may benefit the precancerous lesion and gastric cancer in early stage.
