摘　　要：目的　研究CD44v6蛋白表达在乳腺癌侵袭、转移和预后中的作用。方法　用免疫组化S -P法检测 1 0 4例浸润性乳腺癌石蜡切片中肿瘤细胞CD44v6的表达 ,结合临床和病理资料作分析。结果　CD44v6主要定位于细胞质。CD44v6蛋白表达与浸润性乳腺癌的组织学类型无关 (P >0 .0 5 ) ,与患者年龄大小无关 (P >0 .0 5 ) ,CD44v6在髓样癌、浸润性小叶癌、管状癌、浸润性导管癌、管内为主的浸润性导管癌的阳性率分别为 6 9.2 %、71 .4%、79.2 %、0 %、72 .8%。在年龄>5 0岁和年龄≤ 5 0岁患者的表达为 6 6 .7%、74.3 %。CD44v6与乳腺癌的组织学分级有关 (P <0 .0 5 )。Ⅰ级阳性表达率为 6 4 .5 % ,Ⅱ级为 94.1 % ,Ⅲ级 87.5 %。CD44v6蛋白表达与乳腺癌浸润程度、腋窝淋巴结转移、存活时间有关 (P <0 .0 5 ) ,且随着肿瘤组织浸润程度的增加 ,表达强度增大(P <0 .0 5 )。CD44v6在组织浸润Ⅰ级、Ⅱ级、Ⅲ级的表达阳性率分别为 5 5 .8%、87.5 %、95 .2 %。在腋窝淋巴结转移为 (- )、(+)、(++)的乳腺癌标本中 ,CD44v6蛋白表达的阳性率分别为 2 7.3 %、6 8.8%、89.7%表明CD44v6蛋白表达与腋窝淋巴结转移有关 (P <0 .0 5 )。结论　黏附分子CD44v6的表达与浸润性乳腺癌的浸润、转移、预后有关 。Objective To study the effect of CD44v6 in breast cancer invasion,metastasis and prognosis.Methods by means of S-P test,104 breast cancer specimen in paraffin sections were examined for the expression of CD44v6.Results CD44v6 protein was detected in cancer cells cytoplasm.Its expression was irrelevant with histological type in infiltrating breast carcinoma(P>0.05),The positive rate of CD44v6 expression was 61.5%,85.7%,79.2%,0%,72.8%in medullary carcinoma,infiltrating lobular carcinoma,tubular carcinoma and infiltrating duct carcinoma and infiltrating duct in main inward duct respectively.The expression of CD44v6 was 66.7%,74.3%in the group age>50 years old and in the group age≤50 years old respectively.The expression of CD44v6 was related to the pathological grade of breast cancer(P<0.05).The rate in the first grade was 64.5%,in the second was 94.1%,third was 87.5%.The expression of CD44v6 was also related to the degree of infiltration,axillary lymph nodes metastasis and results of following-up(P<0.05).With the expand of the degree of infiltration,the strength of expression was enlargement.The rate of expression of CD44v6 was55.8%,87.5%,95.2%in tissue infiltration Ⅰ,Ⅱ and Ⅲ;In specimen of axillary lymph nodes metastasis of breast cancer with(-),(+)and(++)was 27.3%,68.8%,89.7%indicated that the expression ofCD44v6 was related to axillary lymph nodes metastasis(P<0.05).Conclusion The expression of CD44v6 has a negative relationship with the type of histology,age,and has a positive relationship with pathological degree,tissue infiltrating,axillary lymph node metastasis,local relapse and distant transfer.Therefore,the expression of CD44v6 can be taken as a reference marker of prognosis in infiltrating breast carcer.
