摘　　要：目的 :观察扩大野体外照射加后程腔内近距离照射治疗食管癌的效果 .方法 :1995 0 1/ 1997 12采用扩大野体外照射配合后程腔内近距离照射治疗中晚期食管癌 92例 (研究组 ) ,与同期单用常规野体外照射治疗 84例 (对照组 )做对比研究 .结果 :①研究组 1,3,5a生存率 (分别为 78.3% ,36 .9% ,2 2 .8% )均高于常规放疗的对照组 (6 5 .4 % ,19.0 % ,6 .9% ) (P <0 .0 5 ) .② 1a的肿瘤局部复发率 ,研究组 (10 .9% )低于对照组 (4 0 .5 % ) (P <0 .0 5 ) .③治疗后食管穿孔的发生率和 1a后良性狭窄的发生率研究组略高于常规放疗组 ,但无明显差异 .④研究组放疗后半程咽食疼痛加重的患者增多 ,但疼痛可以耐受 .结论AIM: To observe the results of extended field external radiotherapy plus intracavitary brachytherapy for esophageal carcinoma. METHODS: From 1995 to the end of 1997, curative effects were compared between two groups: 92 patients with advanced esophageal carcinoma who were treated with extended field external radiotherapy plus intracavitary brachytherapy (study group) and 84 patients who were treated with conventional field external radiotherapy alone (control group). RESULTS: ① The 1,3,5 year survival rates were 78.3%, 36.9% and 22.8% respectively in the study group and 65.4%, 19.0% and 6.9% respectively in the control group ( P <0.05). ② The 1 year local recurrence rates were 10.9% in the study group and 40.5% in the control group ( P <0.05). ③ Although the esophageal perforation rates and the 1 year benign constriction rates seemed higher in the study group than those in the control group, no significant difference was observed. ④ Late course odynophagia was more severe in the study group than that in the control group, but it could be tolerated by most patients. CONCLUSION: Extended field external radiotherapy plus intracavitary brachytherapy is an effective treatment of esophageal carcinoma.
