胃癌nm23、p53、CerbB-2及PCNA的表达与淋巴结转移的关系

摘　　要：目的 探讨胃癌nm23、p53、CerbB-2及PCNA的表达与淋巴结转移的关系。方法 采用免疫组化S-P法检测78例胃癌组织中的nm23、p53、CerbB-2及PCNA的表达。结果 胃癌中nm23、p53、CerbB-2及PCNA的阳性表达率分别是41％、61.5％、39.7％及100％。nm23低表达、p53及CerbB-2过表达、PCNA强阳性表达淋巴结转移率分别是91.3％、93.8％、91.5％、84.6％,均高于nm23过表达、p53及CerbB-2低表达、PCNA弱阳性表达淋巴结转移率(59.3％、53.3％、58.1％、46.2％),差异有显著性(P<0.01)。同时有两个以上指标表达异常者淋巴结转移率为89.8％,明显高于1个指标表达异常和3个指标均不表达者的淋巴结转移率(58.6％),差异有显著性(P<0.01)。结论 nm23、p53、CerbB-2及PCNA在胃癌中的表达与胃癌淋巴结转移密切相关,nm23、p53、CerbB-2的表达在胃癌淋巴结转移中可能起协同作用,可作为临床预测转移及估计预后的重要参考指标。Objective To study the relationship between expressions of nm23,p53,CerbB-2,PCNA and lymph node metastasis in human gastric cancer. Methods Expressions of nm23, p53, CerbB-2 and PCNA in human gastric cancer tissues of 78 cases were observed by immunohistochemical S-P methods.Results The positive rates of nm23, p53, CerbB-2 and PCNA expression were 41%,61.5%, 39.7% and 100 % , respectively, in gastric cancer.The metastatic rates in patients with high level expression of p53,low level expression of nm23 and cerbB-2,strong positive reaction of PCNA(91.3% ,93.8% ,91.5% ,84.6%) were higher than in those with low level expression of p53, high level expression of nm23 and cerbB-2, week positive reaction of PCNA (59.3 % , 53.3 % , 58.1%,46.2%;P<0.01). Meanwhile, the metastatic rates in those with co-expressions of two or three targets were higher than in those with only one or none of expression (89.8% ,58.6%, P < 0.01) . Conclusions The expression of nm23, p53, CerbB-2, PCNA correlates with the lymph node metastasis of gastric cancers. nm23, p53, cerbB-2 may cooperatively play a role in the lymph node metastasis of gastric cancer, which may serve as an important indicator of metastasis and prognosis of gastric cancer.
