羟基喜树碱为主联合化疗治疗晚期消化道肿瘤疗效对照研究

摘　　要：目的 观察使用羟基喜树碱(HCPT)治疗晚期消化道肿瘤的近期疗效及毒性反应。方法 全组晚期消化道肿瘤256例,胃癌131例,大肠癌93例,原发性肝癌21例,食管癌11例。随机分为HCPT治疗组130例和非HCPT治疗的对照组126例。治疗组使用HCPT联合化疗方案,对照组除未用HCPT外,其余各种药物的剂量和用法均与治疗组相同。结果 治疗组总有效率为45.4％,对照组为26.2％(P<0.01),以胃、肠癌效果较好;毒副作用两组相比差异无显著性。结论 HCPT联合化疗可明显提高晚期胃肠道癌的有效率,毒性反应无明显增加,与其他抗癌药无交叉耐药性,提示对晚期、复治的消化道癌患者HCPT是安全有效的药物。Objective To evaluate the efficacy and toxicity of combination chemotherapy with hydroxy-camptothecin( HOT) in advanced digestive tract tumors. Methods There were 256 cases with advanced digestive tract tumors in this study,including 131 gastric carcinoma,93 colorectal carcinoma,21 primary hepatic carcinoma and 11 esophageal carcinoma. They were randomly divided into a study group(l30 cases) and a control group(126 cases) .The patients in the study group were treated mainly with HCPT combined with other anti-cancer drugs, while the patients in the control group were treated by the same anti-cancer drugs as those of the study group except HCPT. Results The overall effective rate of the study group was 45.4%,being significantly higher than that of the control group(26.2% , P < 0. 01) . The efficacies in gastric carcinoma and colorectal carcinoma were especially good.Conclusion The results suggest that curative effects were specially good in advanced digestive tract tumors. The adverse effects were not different significantly between the HCPT group and the control group. Conclusion HCPT is a safe and effective drug for treatment of advanced or recurrent digestive tract tumors.
