关节冲洗术治疗颞下颌关节紊乱病的实验研究

摘　　要：目的 研究关节冲洗对兔颞下颌关节滑膜炎和骨关节病的治疗作用。方法 用基质金属蛋白酶-1腔内注射制备兔的颞下颌关节骨关节病的动物模型,分为滑膜炎组和骨关节病组,分别行左侧关节上腔冲洗治疗,在不同时期观察关节各成分的组织病理学变化。结果 滑膜炎组左侧关节各时期病变均明显轻于右侧。骨关节病组左侧关节各时期病变均与右侧同步,仅程度稍轻。结论 冲洗治疗对兔颞下颌关节的急性滑膜炎有良好的疗效,对早期骨关节病可抑制其病变发展,促进其自身修复。Objective To study the effects of arthrocentesis on rabbit temporomandibular joint synovitis and osteoarthritis. Methods Animal models of temporomandibular joint osteoarthritis were made by injecting matrix metalloproteinase - 1 into rabbit joint cavity, and were divided into synovitis group and osteoarthritis group. Arthrocentesis was applied to the left joint of the two groups. Pathological changes of joint in different periods were observed and analyzed. Results In different periods, pathological changes of the left joint in synovitis group were slightly more than those of the right joint, while the pathological changes of the left joint in the osteoarthritis group were the same as those of the right. Conclusion Arthrocentesis was an effective method for acute synovitis. Arthrocentesis could restrain the pathological changes of osteoarthritis , and improve the reconstruction of TMJ.
