贲门癌高、低发区居民贲门癌前病变和癌组织C-erbB2变化

摘　　要：目的 :探讨贲门癌高、低发区贲门上皮各级病变组织C erbB2蛋白表达的变化特征。方法 :采用组织病理学和免疫组织化学ABC法 ,分析高发区正常贲门上皮 (NOR) 10例 ,慢性浅表性贲门炎 (CSG)组织 2 3例 ,慢性萎缩性贲门炎 (CAG)组织 16例 ,贲门上皮不典型增生 (DYS) 14例 ,贲门癌 (GCA) 70例和低发区NOR 32例 ,CSG组织 38例 ,CAG组织 15例 ,DYS 18例 ,GCA组织 39例中C erbB2蛋白表达情况。结果 :贲门癌高发区从CSG→CAG→DYS→GCA ,C erbB2蛋白阳性表达率分别为 17%、38%、86 %和 81% ;GCA组织中低分化组C erbB2蛋白阳性率 (5 7% )低于高、中度分化组 (92 % ) (P <0 .0 5 )。低发区从CSG→CAG→DYS→GCA ,C erbB2蛋白阳性率分别为 2 1%、2 0 %、5 6 %和 5 1% ;GCA组织中低分化组C erbB2蛋白阳性率 (80 % )高于高、中度分化组 (33% ) (P <0 .0 5 )。结论 :贲门上皮极轻度病变已发生不同程度的C erbB2表达变化 ,并随病变进展而呈现不同程度的升高趋势 ;高发区贲门上皮各级病变的升高趋势 ,特别是DYS ,明显高于低发区居民 ;C erbB2是贲门上皮癌变 。Aim: To further elucidate the possible mechanisms of gastrocardia carcinogenesis, the present study was undertaken to determine the alterations of C erbB2 protein expression in the normal tissues and the tissues with different severity of lesions in gastric cardia from the subjects at high and low incidence area (HIA and LIA) for GCA. Methods: Immunohistochemistry (ABC) method was undertaken to analyses the alterations of C erbB2 protein expression of 10 NOR,23 CSG,16 CAG,14 DYS and 70 GCA from HIA and 32 NOR,38 CSG,15 CAG,18 DYS and 39 GCA from LIA. Results: In the HIA, with the lesions progressed from CSG→CAG→DYS→GCA, the positive rates of C erbB2 expression were 17%, 38%, 86% and 81%, respectively; It's the statistic significance for the C erbB2 protein expression difference between the group of well and moderated differentiated GCA (92%) ( P <0.05) and the group of poorly differentiated GCA(57%). In the LIA, the positive rates of C erbB2 expression from CSG→CAG→DYS→GCA were 21%, 20%, 56%, 51%, respectively; the positive rate of C erbB2 expression in the poorly differentiated GCA (80%) is significant higher than in well and moderate differentiated GCA (33%) ( P <0.05). Conclusion: The results indicated that C erbB2 expression occurred in the very mild cardia lesions, C erbB2 may play an important role in gastrocardia carcinogenesis, especially in early stage.
