摘  要:目的 研究胃组织中bcl 2、p5 3、nm2 3基因的表达与临床病理的关系 ,探讨bcl 2、p5 3、nm2 3基因与胃癌生物学行为的相关性。方法 对 80例胃癌组织 ,37例不典型增生癌旁组织及 2 0例正常粘膜进行免疫组化检测。结果 bcl 2蛋白在癌旁轻、中、重度不典型增生的阳性率分别为 3%、1 9.2 %、2 1 % ,各组间无显著性差异 (P >0 .0 5 ) ,高分化胃癌组织阳性率为 78.2 % ,明显高于低分化胃癌组织的 4 8.0 % (P <0 .0 5 ) ;p5 3在高分化胃癌中阳性率为 5 2 % ,明显低于低分化组的87.5 % (P <0 .0 5 ) ;nm2 3在胃癌组织中阳性表达率为 6 0 % ,明显高于低分化及伴有淋巴结转移胃癌组织 (P <0 .0 5 )。p5 3与bcl 2呈明显负相关。 结论 bcl 2、p5 3和nm2 3的表达不仅与胃癌细胞的分化等级密切相关 。Objective To study the relationship among the expression of p53? bcl 2 and nm23 H1 and clinicopathology in the patients with gastric cancer.Methods 80 cases of different differentiated gastric cancer, 37 cases of peri tumor atypical mucosa hyperplasia and 20 cases of normal gastric mucosa were stained by immunohistochemical SP method.Results The positive rates of bcl 2 protein expression on mild, middle, severe peri tumor atypical hyperplasia were 3.1% , 19.2% , 21.0% .There was no obvious difference ( P > 0.05 ); comparing the positive rate of well differentiated and poor differentiated gastric cancer 78.2% , 48.0% respectively, there was obvious difference ( P < 0.05 ). The positive rate of p53 protein in well differentiated gastric cancer was 52%,obviously lower than that of poor differentiated gastric cancer 87.5% . The total positive rate of nm23 H1 in gastric cancer tissue was 60% The expression rate of nm 23H1 protein was closely correlated to tumor differentiation and lymph node metastasis tissue( P < 0.05 ). There is a negative correlation between p53 and bcl 2 expression.Conclusion bcl 2 and p53 can be one of important index for tumor differentiation and prognosis evaluation. Nm23 H1 protein play a important inhibited role in the process of gastric cancer tissue metastasis and may be a molecular biological basis for the evaluation of patients prognosis in gastric cancer.
