细胞间黏附分子-1表达对肝细胞癌转移和预后的临床价值

摘　　要：目的:分析肝细胞癌(hepatocellular carcinoma,HCC)组织中细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)的表达及其临床价值。方法:选取71例HCC术后癌组织标本作为研究组,另选取43例癌旁肝组织标本作为对照组,应用免疫组化方法,检测两组中ICAM-1的表达,并分析其与临床病理特征的关系。结果:HCC及癌旁组织中ICAM-1均有表达;癌组织ICAM-1的表达呈强阳性染色,主要位于细胞膜和细胞质中,其表达阳性率为71.83%,明显高于癌旁组织(27.91%,χ2=20.90,P<0.001),且表达强度显著高于癌旁组织(Z=-5.191,P<0.001)。HCC中ICAM-1高表达与肝外转移、TNM分期明显相关(P<0.05),而与患者年龄、性别、HBs Ag阳性与否、甲胎蛋白浓度、肿瘤数量、肿瘤大小、有无门脉癌栓和有无肝硬化无关(均P>0.05)。结论:ICAM-1的表达与HCC的进展相关,其异常可作为预测HCC转移和判断预后的指标。Objective:To investigate the expression and significance of hepatic intercellular adhesion molecule-1(ICAM-1) in hepatocellular carcinoma(HCC). Methods: ICAM-1 expressions in 71 cancerous and 43 surrounding non-cancerous liver tissues were detected by immunohistochemistry for analyzing their relationship with clinicopathological features. Results: The ICAM-1 expression was presented in HCC and their non-cancerous tissues. The stronger positive staining of ICAM-1 expression was found in cancerous or their surrounding tissues and was mainly located in cytomembrane and cytoplasm. The positive rate(71.83%) or its intensity were significantly higher(χ2=20.90, P<0.001 or Z=-5.191, P<0.001) than those in their surrounding tissues(27.91%). The expression of ICAM-1 was significantly related to extra-hepatic metastasis and tumor-node-metastasis(TNM)stage(P<0.05), without significant different between ICAM-1 expression and age or sex, positive-HBsAg, alpha fetoprotein, tumor number, tumor size, portal vein tumor thrombus, or liver cirrhosis(P>0.05). Conclusions:Expression of ICAM-1 is associated with occurrence and development of HCC and one of useful molecular markers to predict the metastasis and prognosis of HCC.
