摘  要:目的 通过了解原发性肝癌及其癌旁组织的表达情况 ,初步探讨重组人生长激素(rhGH)在肝癌患者中的适用问题。方法 采用放射配体法对 3 2例原发性肝癌 (HCC)组织及其癌旁组织进行生长激素受体 (GHR)的检测 ,并以 6例正常肝组织作为对照 ;肝组织GHR的受体最大结合量 (RT)和平衡解离常数 (Kd)采用Scatchard法计算 ;并分析肝癌GHR受体结合容量 (RT)值与肝癌临床病理特点之间的关系。结果 在 2 8例HCC组织与 3 2例癌旁组织检测到GHR的存在 ,RT 在有GHR表达的肝癌为 ( 18.43 0 0± 4.163 3 )fmol·mg-1·protein ,平衡解离常数 (Kd)为( 0 .64 3 2± 0 .1961)nmol/L ,癌旁组织GHR的RT 值为 ( 3 2 .744 1± 3 .5 10 6)fmol·mg-1protein ,Kd为 ( 0 .610 9± 0 .110 5 )nmol/L ,同正常肝组织相比 ,肝癌与癌旁组织GHR的RT 值均降低 (P <0 .0 5 ) ,而肝癌组织GHR的RT 又低于癌旁组织 ,但三者之间Kd 差异无显著性 (P >0 .0 5 ) ;肝癌组织GHR的RT 值与肿瘤大小、患者临床分期的延迟呈负相关 ,而与肿瘤分化程度、患者年龄、是否合并肝硬化无关 ;有 4例肝癌组织未检测到GHR存在。结论 大多数原发性肝癌组织及其癌旁组织均表达GHR ,在它们受体功能未明的情况下 ,rhGH在肝癌患者中的使用可能需慎重。Objective To preliminarily study if the recombinant human growth hormone (rhGH) is suitable for hepatocellular carcinoma (HCC) treatment by investigating the expression of growth hormone receptor (GHR) in hepatocellular carcinoma (HCC) and adjacent liver tissues.Methods Radioreceptor assays were used to detect GHR in 32 HCC and adjacent liver tissues.Six normal liver tissues were studied as controls.Receptor binding capacity (R T) and affinity constant (K d) of GHR were calculated by Scatchard's method,and the relationship between cancer GHR's R T and clinicopathologic factors was also analyzed.Results The growth hormone-specific singular binding site,namely GHR,was detected respectively in 28 HCC cases and 32 adjacent liver tissues.The GHR's binding capacity (R T) was (18.430?0±4.163?3) fmol/mg protein and the affinity constant (K d) was (0.643 2±0.196?1) nmol/L in HCC tissues.In the adjacent liver tissues R T was (32.744?1±3.510?6) fmol/mg protein and K d was (0.610?9±0.110?5) nmol/L.R T in both HCC and adjacent tissues was lower ( P <0.05) than in the normal liver tissue.At the same time,RT in adjacent tissues was higher than in HCC tissues,but K d didn't show any difference ( P >0.05).The R T of the GHR was negatively related to the tumor's size and disease stage,but had little to do with differentiation of tumor,ages of the patients or whether the patient suffered from cirrhosis at the same time.GHR was absent in 4 HCC tissues.Conclusion Most of the HCC tissues still express low level of GHR.Before their functions are well understood,rhGH should be very carefully used in HCC patients.
