摘 要:目的:观察全视网膜光凝术(panretinal photocoagulation,PRP)对增殖性糖尿病视网膜病变(PDR)的疗效、影响疗效的因素及激光量的选择。方法:对1998~2000年门诊随机治疗的符合多中心糖尿病视网膜病变激光治疗组(DiabeticRetinopathy Photocoagulation Study Group,DRPSG)制定的标准362例(551只眼)PDR患者PRP术后情况进行观察。并与对照组进行对照。结果:从两组结果可以看出治疗组比对照组用的光斑大,点数少,光斑反应重,范围广,疗效好。结论:根据视网膜病变的严重程度选择不同的PRP技术是治疗成功的关键之一。适宜波长的激光、光凝范围、光斑密度,光斑大小、光照时间、输出功率三个技术参数的合理应用,以最少的点数达到最佳的治疗效果。Objective:To observe the therapeutic effects, the affecting factors and the choice of laser power of panretinal photocoagulation (PRP) in treating proliferative diabetic retinopathy (PDR). Method:From 1988 to 2000 in outpatient department,362 patients (551 eyes),conformed to the standard of Diabetic Retinopathy Photocoagulation Study Group (DRPSG), were treated with PRP and compared with the contral group. Results: In the treatment group, bigger photocoagulation patch, less photocoagulation points and more considerable extent were more effective than those in the central group. Conclusion:Choosing various PRP technique according to the severity of retinopathy is one of the keys to the success of PRP technique. Only rational use of the three technique parameters. Proper wave length of laser,extent of photocoagulation,density of photocoagulation patch and photocoagulation time and electricity power one can get the best curable effect with the least laser points.
