改良根治术治疗乳癌95例回顾性分析

摘　　要：目的 :探讨改良根治术治疗乳癌的疗效。方法 :对采用改良根治术 95例 (MRM组 )和Halsted根治术 177例 (RM组 )的乳癌患者的临床资料进行回顾性分析和对比。结果 :全部 2 72例均获随访 (MRM组 95例 ,RM组 177例 ) ,随访 3～ 14年。随访 3年以上的 2 72例 (MRM组 95例 ,RM组 177例 ) ,5年以上的 2 14例 (MRM组 5 5例 ,RM组 15 9例 ) ,10年以上的 6 7例 (MRM组 30例 ,RM组 37例 )。MRM组和RM组 3、5、 10年生存率分别为 10 0 %、 85 71%、 70 97%和 95 6 0 %、 83 13%、 71 0 5 % ;3、 5、 10年局部复发率分别为 4 0 8%、 7 1%、 6 4 5 %和 3 89%、 6 88%、 5 2 6 % ,3、 5、 10年远处转移率分别为 6 10 %、 12 5 0 %、12 90 %和 6 6 7%、 11 2 5 %、 15 79% ,两组各指标均无显著差异 (P >0 0 5 ) ,而MRM组患者上肢活动功能较RM组明显为佳 (P <0 0 1)。结论 :改良根治术治疗乳癌 ,其根治性疗效同Halsted根治术 ,而患侧上肢功能好 ,并发症少 。Objective:To evaluate the effect of modified radical mastectomy on breast cancer (BC).Methods:The clinical data of 95 cases of BC treated with modified radical mastectomy (group MRM) and 177 cases of BC treated with halsted radical mastectomy (group RM) were retrospectively analyzed and compared.Results:All of the 272 cases had been followed up for a period from 3 years to 14 years.All of the 272 cases were followed up for more than 3 years (group MRM 95 cases,group RM 177 cases),214 casesfor more than 5 years (group MRM 55 cases,group RM 159 cases),67 cases for more than 10 years (group MRM 30 cases,group RM 37 cases).In group MRM and group RM,the 3 ,5 and 10 year local recurrence rate were 4 08%,7 10%,6 45% and 3 89%,6 88%,5 26%,the 3 ,5 and 10 year distant metastasis rate were 6 10%,12 50%,12 90% and 6 67%,11 25%,15 79% respectively ( P >0 05).The upper extremity function in group MRM was better than that in group RM( P <0 01).Conclusion:Modified radical mastectomy for BC has similar effect as Halsted operation.Compared with group RM,patients in group MRM have good function of upper extremity,and less complications,is an ideal method in treating breast cancer.
