颈部吻合治疗胸段食管癌175例临床分析

摘　　要：目的 :总结 1 75例胸段食管癌的外科治疗结果 ,探讨食管癌的临床特点及术式选择。方法 :1 6 8例患者左侧开胸经食管床食管胃左侧颈部单层宽边吻合术 ,7例右侧开胸经食管床食管胃左或右侧颈部单层宽边吻合术。结果 :全组手术治愈 1 72例 ,死亡 3例 ;发生吻合口瘘 1 5例、胸胃壁瘘 2例 ,乳糜胸 4例和幽门梗阻 1例 ,均获治愈。上切缘癌阳性 5例 ,其中中段癌 4例 ,上段癌 1例 ;下切端癌阳性 3例 ,均为下段癌。中段食管癌行颈部吻合上切端癌阳性率较弓上吻合差异有非常显著性 (P <0 0 1 )。结论Objective:To review the results of cervical esophagogastrostomy in 175 cases patients with thoracic esophageal carcinoma.Methods:168 cases patients were performed cervical esophagogastrostomy through left thoracic and 7 cases through right thoracic.Results:172 cases recovered by operation and there were 3 early deaths postoperatively. 15 patients had anastomotic fistula, 2 patients had chest wall stomach fistula, 4 patients had chylothorax, and 1 patient had pyloric obstruction. 5 patients had positive of upper cutting margin all in down portion of esophagus. For middle portion of esophageal carcinoma, the positive of upper cutting margin with upper aortic arch esophagogastrostomy have rate of cervical esophagogastrostomy( P <0 01).Conclusion:Cervical esophagogastrostomy is the first preferrable and main treatment for thoracic esophageal carcinoma.
