摘  要:目的 观察和探讨原田病 (Harada disease)的光学相干断层扫描 (optical coherence tomography,OCT)的图像特征。方法 对 2 3例 (4 6只眼 )原田病患者同时进行荧光素眼底血管造影 (fundus fluorescein angiog-raphy,FFA )和 OCT检查 ,并对所获得的图像进行分析。结果 原田病的 OCT表现为 :急性期 :(1) 2 6只眼表现为后极部多发性、多灶状视网膜神经上皮层脱离 ;(2 ) 12只眼表现为黄斑部神经上皮层间水肿 ,中心凹处神经上皮层脱离 ,中心凹隆起 ,结构不易辨认 ;(3 ) 8只眼表现为黄斑部大泡样视网膜神经上皮层脱离 ,其下方的色素上皮及脉络膜反光带显著变薄 ,呈波浪状 ;(4 ) 3 2只眼表现为视乳头周围视网膜水肿增厚。结论  OCT断层扫描能直观的、准确的呈现原田病的损害程度。对视网膜组织细微结构的改变及视网膜继发性损害的程度 ,提供了一个直观的二维断层扫描图像 ,对Objective To observe and study the characteristics of Harada disease by optical coherence tomography(OCT) scanning.Methods 23 patients of harada disease were included in the study.An OCT scanning along with a fundus fluorescein angiography (FFA) were performed in all patients.Resulted images were analyzed.Results The OCT characteristics of the Harada disease were seen as follows:in acute stage:①26 eyes had the multiple retinal neurosensory detachment in the posterior pole;②12 eyes had intra-neurosenory edema within the macular fovea,the reflect light of the inner limiting membrane could not be seen,a neurosensory detachment can be seen at the fovea,and the structure of the macular fovea was hard to be recognized.③8 eyes had big bubble like retinal neurosensory detachment.④32 eyes had an uneven increase in the thickness of neuroretina around the optic nerve head.Conclusions OCT can directly and accurately detect the retinal damages caused by the Harada disease.It gives a direct 2 dimension image that can provides additional information to FFA in showing the tinny,and secondary structural changes occurred in the neuroretina.
