摘 要:目的 :评价肝癌患者体内树突状细胞的功能与状态 ,为肝癌DC疫苗的研制奠定基础。方法 :改进目前国内外普遍使用的DC诱导方法 ;用改进后的DC诱导方法 ,诱导肝癌患者DC ,检测DC表面各种分子的表达及其诱导同种异体淋巴细胞转化的能力和DC培养上清中TNF和IL - 1 2水平。结果 :经GM -CSF和IL - 4联合诱导 ,平均从 1 0 0ml健康人外周血中诱导出 9.7× 1 0 6 个DC ,诱导出 9.2× 1 0 6 个CD83+ 成熟DC ,CD83分子表达明显高于国内报道 ;健康人DC诱导同种异体淋巴细胞转化的能力高于肝癌患者 ,但肝癌患者和健康人DC培养上清中IL - 1 2含量几乎没有差别 ,反而肝癌患者DC培养上清中TNF水平高于健康人。结论 :肝癌患者自身的DC仍具有一定的抗原呈递能力 ,适当调控有可能使其行使APC功能 。Objective:To evaluate the function and state of dendritic cells (DCs) in hepatocellular carcinoma (HCC)patients and to lay down a foundation for preparing the vaccine against HCC.Methods : With modified methods ,we get high purity of DCs by culturing plastic-adherent monocytes isolated from healthy human peripheral blood with GM-CSF and IL-4 for 7 days. The allogeneic mixed lymphocytes reaction of DCs and the levels of IL-12 and TNF in culture medium of DCs in hepatocellular carcinoma patients were also detected.Results:DCs induced with our modified methods expressed both CD83 and CD14 molecules highly , and had a higher density than those in other domestic reports. After cultured for 7 days ,CD14 + monocytes differentiated into high purity of DCs with a density of 9.7×10 6 per 100 ml peripheral blood which included of 9.2×10 6 of CD83 + mature DCs . These DCs displayed all phenotypic and morphologic characteristics of mature dendritic cells and were more potent in stimulating T lymphocytes than these from patients with HCC. There was no difference at the levels of IL-12 in culture medium between normal and patient, but the levels of TNF in patients were higher than those in normal.Conclusion : The higher level of TNF in DCs culture medium of HCC patient suggest their DCs still have certain antigen presenting ability and would improve it by optimizing the culture conditions and have a potential value in designing and application of individual vaccine .
