摘 要:目的 探讨 β- catenin在结直肠肿瘤发生、发展中的作用及临床意义。 方法 采用免疫组织化学方法检测 5 5例结直肠腺癌、33例结直肠腺瘤组织中 β- catenin基因蛋白产物的表达水平。 结果 β- catenin在结直肠腺癌组织阳性表达率 4 5 / 5 5 (81.8% ) ,结直肠腺瘤组织阳性表达率 2 3/ 33(6 9.7% ) ,显著高于正常粘膜组织 (0 ) ,差异有显著性 (P<0 .0 5 )。低分化腺癌阳性表达率 (13/ 13)明显高于中、高分化结直肠腺癌 (19/ 2 3,13/ 19,P<0 .0 5 )。伴淋巴结转移阳性表达率 (2 5 / 2 7,92 .6 % )明显高于不伴淋巴结转移 (2 0 / 2 8,71.4 % ,P<0 .0 5 )。Dukes C、D期结直肠腺癌阳性表达率 (31/ 34,91.2 % )明显高于 A、B期 (14 / 2 1,6 6 .7% ,P<0 .0 5 )。β- catenin阳性表达与结直肠肿瘤的大小、部位、患者的年龄、性别无关。 结论 β- catenin在散发性结直肠癌的发生发展过程中起重要作用。检测肿瘤组织 β-Objective To explore the role of β\|catenin in the carcinogenesis and progression of sporadic colorectal adenocarcinoma. Methods Immunohistochemistry stainings were used to detect the expression of β\|catenin protein in 45 cases of normal colorectal mucosa, 33 cases of sporadic colorectal adenoma, 55 cases of sporadic colorectal carcinoma. Results (1)The percentages of β\|catenin expression in the colorectal adenocarcinomas(88%) and adenomas(69 7%) were significantly greater than that in normal mucosa tissues(0, P<0 05). (2)The positive staining rate of β\|catenin expression in moderate, well differentiated colorectal adenocarcinomas(19/23, 13/19 respectively) was less than that in the poorly differentiated ones(13/13, P<0 05). The percentage of β\|catenin expression in lymphonode metastasis group(92 6%) was greater than that in non\|lymphonode metastasis group(71 4%, P<0 05). The nuclear positive rate of β\|catenin protein expression in the Dukes' A and B stage group were lower than that in Dukes' C and D stage group. There were no significant differences in the incidence of β\|catenin protein expression among different age, sex groups and different site, size of the colorectal adenocarcinomas groups. Conclusion (1)β\|catenin protein plays an important role in carcinogenesis of sporadic colorectal adenocarcinomas. (2)The detection of the expression of β\|catenin in colorectal carcinomas is valuable for evaluating the biological behavior of colorectal carcinomas.
