MUC1粘蛋白在进展期胃癌中的表达及临床意义

摘　　要：目的　检测MUC1粘蛋白在进展期胃癌中的表达 ,探讨其对胃癌预后的意义。方法　采用免疫组化SP法 ,应用抗MUC1粘蛋白核心蛋白抗体 (DF3抗体 )检测 6 0例胃癌根治标本及对照组 2 0例胃正常黏膜组织标本MUC1粘蛋白表达。采用 χ2 检验及Kaplan Meier生存曲线进行统计分析。结果　MUC1粘蛋白阳性表达率 5 5 %(33 6 0 )。有淋巴结转移胃癌组MUC1粘蛋白阳性表达率为 71.0 % (2 2 31) ,无淋巴结转移组MUC1粘蛋白阳性表达率为 37.9% (11 2 9) ,组间差别显著 (P <0 .0 5 )。按PTNM分期 ,Ⅰ期胃癌的MUC1粘蛋白阳性表达率为 2 0 % ,Ⅱ期表达率 6 8.8% ,Ⅲ期表达率 5 3.3% ,Ⅳ期表达率 78.6 % ,Ⅰ期与Ⅱ、Ⅲ、Ⅳ期间存在显著统计学差异 (P <0 .0 5 )。经Kaplan Meier法生存曲线统计表明 ,MUC1阳性表达组 5年生存状况较MUC1阴性组差 (Log Rank检验 ,P =0 .0 37)。结论　在进展期胃癌中MUC1粘蛋白表达与淋巴结转移相关 ,随着胃癌的进展MUC1表达呈上升趋势 ;Objective To deterimine the expression of MUC1 in the tissue of advanced gastric carcinoma and to probe its relation to prognosis of the patients.Methods Immunohistochemical studies for the expression of MUC1 core peptide antigen which was detected by antibody DF3 in 60 specimens of advanced gastric carcinoma and 20 specimens of normal gastric tissue which was used for control group. The data were analysed by chi square test, Kaplan Meier method and Log Rank test.Results The total expression rate of MUC1 was 55% (33/60) in advanced gastric carcinoma. The expression rate 71%(22/31) of MUC1 in the group with positive lymph node metastasis was significantly higher than that 37.9%(11/29) in the group with negative lymph node metastasis ( P <0.05).The expreesion rates of MUC1 inⅠ stage,Ⅱ stage,Ⅲ stage and Ⅳ stage were 20%, 68.8%, 53.3% and 78.6%, respectively,and the expression rate of MUC1 inⅠ stage was significantly lower than that in other stages( P <0.05). According to Kaplan Meier method, the five year survival of the patients with positive MUC1 expression was worse than that with negative MUC1 expression (Log Rank test, p=0.037).Conclusion In advanced gastric carcinoma, the positive MUC1 expression is associated with lymph node metastasis. With the gastric carcinoma developing,the express of MUC1 trends to increasing. The positive MUC1 expression is related to poor prognosis of patients.
