单疱病毒角膜炎新分类法评价

摘　　要：目的 :评价Holland等提出的单疱病毒角膜炎 (HSK )新分类法在指导临床诊治HSK的意义。方法 :应用Holland等的新分类法 ,诊断及治疗门诊各类型单疱病毒角膜炎患者 2 0 4例 ,观察其诊断及治疗效果。本组未包括神经营养性角膜病。结果 :本组 2 0 4例HSK中 ,属于感染性上皮角膜炎 86例 ,其中角膜小泡 13例 ,树枝状溃疡 3 3例 ,地图状溃疡 3 8例 ,缘部溃疡 2例 ,全部治愈。属于基质性角膜炎 91例 ,其中坏死性基质角膜炎 2例 ,药物治疗无效 ,行穿透性角膜移植术。免疫性基质角膜炎 89例 ,全部治愈。属于角膜内皮炎 2 7例 ,其中中央型 13例 ,周边型 8例 ,弥漫性 6例 ,全部治愈。结论 :Holland等的新分类法可指导临床诊治HSK ,提高临床诊治质量 ,改善预后。Objective:To evaluate the clinical value of the new classification system by Holland in directing the diagnosis and treatment of Herpes Simplex Virus (HSV) keratitis. Methods:The diagnosis and treatment of 204 cases of every types of HSV keratitis were analyzed according to the new classification system of Holland.Neurotrophic keratopathy was not included in the study. Results:In 204 cases of all types of HSV keratitis,86 cases were infectious epithelial keratitis that was cured completely,including 13 cases of cornea vesicle,33 cases of dendritic ulcer,38 cases of geographic ulcer and 2 cases of marginal ulcer.91 cases were stromal keratitis,including 2 cases of necrotizing stromal keratitis that was performed penetrating keratoplasty due to no response to medications.89 cases were immune stromal keratitis that was all cured.27 cases were endotheliitis that were cured,including 13 cases of disciform endotheliitis, 8 cases of linear endotheliitis and 6 cases of diffuse endotheliitis. Conclusions:The new classification system by Hollard may improve the diagnosis and treatment of HSK keratitis.
