中老年人自然头位头影测量的重复性研究

摘  要:目的:探讨头影测量片研究自然头位(natural head posture,NHP)的方法,分析中老年个体自然头位的可重复性。方法:选取 20名40-70岁的中老年个体,用头影测量法记录其自然头位。在不干扰自然头位的条件下,放入辅助定位耳塞, 拍摄头影测量片。对其中10名受试者重复投照, 分别放入耳塞和不放入耳塞。对另外10名受试者重复投照时均放入耳塞。采用扫描仪和计算机测量NSL/VER角和N-Me距离, 分析测量的可靠性和NHP的重复性。结果:耳塞干扰性实验中,放与不放耳塞投照的侧位片NSL/VER角度差异没有显著性(p>0.05),Se为2.59°。用耳塞投照的侧位片头影测量角度和线距的测量误差较小(Se=0.33°,Se=0.28mm)。自然头位重复投照测量值的差异没有显著性,Se为0.86°。结论:在条件控制良好的情况下,中老年个体的NHP是高度可重复的。在多次投照NHP的头颅定位侧位片时放入耳塞,可使投照的重复性更好。Objective: To probe into the studying method of natural head posture(NHP) and to analyze the reproducibilityof NHP by means of cephalometric technique. Methods: Twenty subjects aged from 40 to 70 took part in the test, in whichNHP was recorded by means of cephalometric technique and measured by radiographs with a scanner and a computer. Thedisturbing effect of ear post was studied in 10 subjects and the reproducibility of NHP was studied in the other 10 subjects.Results: No significant difference in reproduibility was between NHP recordings taken with and without ear posts(p>0.05,Se=2.59?. However, without ear posts the quality of radiographs ( Se=1.57? Se=1.37mm) tended to be lower than thosewith ear posts ( Se=0.33? Se=0.28mm). There was no significant difference in the repositioning exposure and the error ofreproducibility of cephalometric technique was 0.86? Conclusion: NHP of middle-aged people or elders was highlyreproducible under carefully controlled experimental condition by use of cephalometric technique. Lateral cephalometricradiographs exposed with ear posts were of good quality.
