摘  要:目的 :探讨溃疡性结肠炎患者肠道菌群改变及机体免疫反应。方法 :采用梯度稀释法进行菌群分析 ,同时采用透射比浊法测定IgG、IgM、IgA。 结果 :发现服用培菲康的溃疡性结肠炎患者急性期菌群与正常人相比 ,肠杆菌 (8 82± 0 6 9,P <0 0 5 )、肠球菌 (7 73± 0 2 1,P <0 0 1)及小梭菌 (6 6 8± 0 78,P <0 0 1)的数量显著增加 ,而双歧杆菌 (6 89± 0 34,P <0 0 1)和乳杆菌 (6 95± 0 5 2 ,P <0 0 1)的数量明显下降 ,且缓解期与对照相比差异无显著性。拟杆菌 (7 2 6± 0 0 3,P <0 0 5 )及双歧杆菌 (7 5 9± 0 34,P <0 0 1)较急性期明显上升 ,小梭菌 (6 13± 0 6 6 ,P <0 0 1)明显下降。未服用培菲康的溃疡性结肠炎患者急性期菌群中服药前后差异无显著性。治疗前肠杆菌 (8 77± 0 89,P <0 0 1)和肠球菌 (7 75± 0 38,P <0 0 1) ,小俊菌 (6 6 4± 0 4 3,P <0 0 1)明显增加 ,而双歧杆菌 (6 98± 0 2 5 ,P <0 0 1)和乳杆菌 (6 83±0 6 2 ,P <0 0 1)明显下降。治疗后肠杆菌 (8 93± 0 6 0 ,P <0 0 1) ,肠球菌 (7 6 1± 0 39) ,小梭菌 (6 6 8±0 72 ) ,有所下降 ,双歧杆菌 (7 12± 0 35 )和乳酸杆菌 (6 6 8± 0 72 )有所上升 ,但治疗前后差异无显著性。Objective:To investigate The changes of fecal flora and humoral immunofunction with ulcerative colitis patients.Methods:The fecal flora is analyzed by gradient dilution method and we measured the humoral immunofunction of them by transmission turbidmentry at the same time. Results: The Enterobacteria (8.82±0.69, P <0.05)? Enterrococcus (7.73±0.21, P <0.01) and Clostridium (6.68±0.78, P <0.01) increased significantly in acute UC patients compared with controls,while Bifidobacterium (6.89±0.34, P <0.01) and Lactobacillus (6.95±0.52, P <0.01) reduced sharply.At the inactive phase of UC patients,no great changes contrasted with the controls. Bacteroides (7.26±0.03, P <0.05) and Bifidobacterium (7.59±0.34, P <0.01) ascended significantly at the inactive phase of UC than that of active phase. Clostridium (6.13±0.66, P <0.01) decreased simultaneously.Comparison with the group with using BIFICO,there were increases in Bifidobacterium and Bacteroides ,yet Clostridium decreased after using BIFICO.At the same time,there is a disorderin humoral immunofunction.At the acute phase of IBD,IgG increased greater than that of controls,and IgA escalated at inactive phase.Conclusion:The results can provide a scientific basis for clinical therapy on the ulcerative colitis,and might assess the prognosis of it.
