多层面螺旋CT仿真膀胱镜对膀胱肿瘤的诊断价值

摘　　要：目的 :探讨多层面螺旋CT仿真膀胱镜 (multi slicespiralCTvirtualcystoscopy ,MSCTVC)对膀胱肿瘤的诊断价值。方法 :对以血尿为主诉 ,经B超发现膀胱内占位性病变的 2 0例患者行MSCTVC和传统膀胱镜检查。通过导尿管将 4 .8%泛影葡胺注入膀胱 ,行盆腔CT扫描 ,将资料传至工作站 ,采用Voyager软件angioscopy模式行仿真膀胱镜影像重建。结果 :传统膀胱镜证实的 2 5处膀胱内病变MSCTVC显示 2 3处 ,准确率 92 %。输尿管口和尿道内口均清晰显示。结论 :MSCTVC具有重建图像清晰逼真、大视野、无盲区等特点 ,能较准确地发现膀胱内占位性病变 ,可作为尿道狭窄。Purpose:To assess the diagnostic value of multi-slice spiral CT virtual cystoscopy (MSCTVC) in bladder tumor. Methods:MSCTVC and conventional cystoscopy were performed in 20 cases hematuria patients in whom bladder tumor was suspected by ultrasonography. After filling the bladder with 300～500 ml of diatrizoate meglumine( 4.8 %) through a 12Fr balloon catheter, CT scanning was underwent, and the data was transmitted to the workstation, where 3D virtual cystoscopy images were generated using the Voyager soft ware.Results:Of the 25 masses in the bladder confirmed by conventional cystoscopy, MSCTVC detected 23 masses with diagnostic accuracy of 92%, which included 22 cases of bladder cancers and 1 case of carcinoma of ureter. The Ureteral orifices and internal urethral orifice were clearly displayed. Conclusions:Virtual cystoscopy image was clear and the visual field was wide without blind area. For these characteristics, MSCTVC can find bladder masses accurately, and it may serve as an alternative to conventional cystoscopy in selected cases, e.g., patient with urethral stricture, serious hematuria, or refusal of conventional cystoscopy.
