透明质酸钠在小梁切除手术中的应用

摘　　要：目的评价透明质酸钠在小梁切除手术中的应用效果。方法随机选取青光眼患者115例124眼,其中65眼为治疗组,术中应用透明质酸钠;59眼为对照组,行常规的小梁切除手术。术后对视力、眼压、前房及滤过泡情况进行分析和统计,平均随访时间15.2±5.6个月。结果术后浅前房治疗组发生10眼(15.38%),无浅Ⅲ度病例;对照组术后发生浅前房19眼(32.20%),其中浅Ⅲ度4眼(6.78%),2例经脉络膜上腔放液后前房重建,统计学分析两组差异有显著性(P<0.05)。而在眼压、视力分布、滤过泡形态和滤过性手术失败的眼数经统计学处理差异均无显著性。结论小梁切除手术中应用透明质酸钠,明显降低了术后浅前房的发生率,避免严重并发症,但对小梁切除手术远期的影响效果仍需进一步观察。Purpose To evaluate the effect of sodium hyaluronate used in the trabeculectomy. Methods 115 patients ( 124 eyes) with glaucoma were divided into two groups randomly, the sodium hyaluronate were used in trabeculectomy in one group (65 eyes) and not used in the other group (59 eyes). The postoperative intraocular pressure (IOP), the visual acutiy,the anterior chamber and the shape of filtering bleb were anlaysed and statistically compared. The average follow-up period was 15.2±5.6 months. Results The rate of complications of shallow anterior chamber in treatment group using sodium hyaluronate was 15.38% (10eyes) and that of the control group was 32.20% ( 19 eyes) ( P < 0.05) , but there were no statistical differences in the two groups in the postoperative IOP, the visual acuity, the shape of filtering bleb and the number of failure in filtering surgery. Conclusion Sodium hyaluronate used in filtering operation can decrease the complications and elevate the safety.
