大肠癌中nm23基因产物的表达与其侵袭转移的关系

摘　　要：目的 :探讨大肠癌 nm2 3基因蛋白 (nm2 3- H1 )的表达水平与大肠癌浸袭、转移的关系。方法 :应用免疫组织化学法对 73例大肠癌及其同源正常 (癌旁 )黏膜组织和 1 6例大肠腺瘤 nm2 3- H1 基因蛋白的表达水平进行研究。结果 :大肠癌中 nm2 3- H1 基因蛋白表达水平明显低于癌旁正常肠黏膜组织和良性腺瘤 (均 P <0 .0 0 1 ) ;转移癌 nm2 3- H1 蛋白表达水平明显低于原发癌 (P<0 .0 0 1 ) ;大肠癌中 nm2 3- H1 蛋白的表达与大肠癌组织分化、肠壁的浸润程度、临床分期、转移均有密切关系 (P <0 .0 0 1 ) ;nm2 3- H1 基因蛋白高表达组术后预后明显优于低表达组 (P <0 .0 0 1 )。结论 :nm2 3- H1 基因产物表达降低可能是大肠癌浸袭、转移的重要原因之一 ,检测大肠癌组织中的 nm2 3- H1Objective:In order to explore the expresson level of nm23 gene product(nm23 H 1 protein)and its relationship with invasion and matastasis of colorectal carcinoma.Methods:The expression level of nm23 H 1 protein was determined in 73 colorectal carcinoma and 16 adnoma immunohistochemically. Result:The positive rate of nm23 H 1 protein expression in colorectal carcinoma was significantly lower than that in adjacent normal mucose and adenoma(P<0.001,P<0.001).The positive rate of nm23 H 1 protein expression level in metastatic cancer was significantly lower than that in primary cancer(P<0.001). There were significant differences of the positive rate of nm23 H 1 protein in colorectal carcinoma between histological type,depth of invasions,Duke's term and the tumour metastasis(P<0.001). The 5 year survival rate of patients wiht high expression of nm23 H 1 protein were significantly higger than those with low expression of nm23 H 1 protein.Conclusion:The decrease of nm23 H 1 protein may be a key cause of colorectal carcinoma metastasis. Determination of nm23 H 1 protein expression is the most reliable test to predict metastasis and prognosis in patients with colorectal carinoma.
